摘  要:目的:比较二维超声平面显像与实时三维经食管超声心动图(3DTEE)在显示房间隔缺损(ASD)形态学特征方面的优缺点和应用价值。方法:对87例ASD患者分别应用二维超声平面显像、3DTEE显示缺损平面图像,比较两者ASD平面显示成功率、ASD最大径测量,并将两者测量ASD最大径与封堵器大小行相关性分析。结果:二维超声平面显像法ASD平面显示成功率为67.8%(59/87),3DTEE法为87.4%(76/87),差异有统计学意义(P<0.001)。二维超声平面显像法测得ASD最大径为(21.30±6.06)mm,3DTEE法为(20.90±6.12)mm,差异无统计学意义(P>0.05),两种方法测得ASD最大径与封堵器大小呈显著正相关(r=0.934、0.975,P<0.001)。二维超声平面显像法显示ASD平面失败主要为小单孔型ASD、多孔型ASD;3DTEE法显示ASD平面失败主要为大单孔型ASD、原发孔型ASD。结论:3DTEE为显示ASD形态学特征的理想手段,二维超声平面显像可作为补充。Objective: To campare the advantages and disadvantages of en face imaging with two-dimensional echocardiography and three-dimensional transesophageal echocardiography(3 DTEE) in imaging morphological characteristics of atrial septal defect(ASD). Methods: En face imaging with two-dimensional echocardiography and 3 DTEE were applied to 87 patients with ASD to display defect planar images. The two ASD planes showed the success rate and the ASD maximum diameter measurement were compared, and the correlation between the two measurements of the ASD maximum diameter and the size of the occluder was analyzed. Results: The ASD plane display success rate of the En face imaging with two-dimensional echocardiography method was67.8%(59/87), and that of the 3 DTEE method was 87.4%(76/87). The maximum diameter of the ASD measured by En face imaging with two-dimensional echocardiography method was(21.30±6.06) mm, and the 3 DTEE method was(20.90±6.12) mm, and there was no statistically significant difference between the two groups(P>0.05). The maximum diameter of ASD measured by the two methods was significantly positively correlated with the size of the occluder(r=0.934, 0.975, P<0.001). En face imaging with twodimen sronal echocardiography method showed that the failure of ASD plane was mainly small single-hole type ASD and porous type ASD. The 3 DTEE method showed that the plane failure of ASD was mainly large single-hole type ASD and primary-hole type ASD. Conclusion: 3 DTEE is an ideal means to display the morphological characteristics of ASD, and En face imaging with two-dimensional echocardiography can be used as a supplement.
