超脉冲二氧化碳点阵激光治疗痤疮萎缩性瘢痕的临床价值

摘　　要：目的分析超脉冲CO_(2)点阵激光治疗痤疮萎缩性瘢痕的效果和对患者生活质量的改善情况。方法回顾性分析2018年1月至2020年6月收治的确诊为痤疮萎缩性瘢痕200例患者的临床资料,根据治疗方法的不同分为观察组和对照组,每组100例。观察组采取超脉冲CO_(2)点阵激光治疗,对照组采用皮肤磨削术治疗。采用t检验比较两组患者治疗前后痤疮瘢痕权重(ECCA)评分以及生活质量评分;采用χ^(2)检验比较两组的临床疗效以及不良反应发生情况。结果观察组的临床疗效总有效率为92.0%,明显高于对照组的84.0%,差异有统计学意义(χ^(2)=5.125,P<0.05);治疗前,两组ECCA评分与生活质量评分对比,差异无统计学意义(t值分别为1.254、1.174,P值均>0.05);治疗后,观察组ECCA评分与生活质量评分明显高于对照组,差异有统计学意义(t值分别为5.412、5.326,P值均<0.05);观察组治疗不良反应发生率为31.0%,明显低于对照组的37.0%,差异有统计学意义(χ^(2)=5.374,P<0.05)。结论超脉冲CO_(2)点阵激光治疗痤疮萎缩性瘢痕较皮肤磨削术具有更好的治疗效果与安全性,对于降低不良反应率和改善患者生活质量具有重要的临床价值。Objective To analyze the effect of ultra pulse CO_(2) dot matrix laser on acne atrophic scars and the improvement of patients´quality of life.Methods The clinical data of 200 patients with acne atrophic scar treated from January 2018 to June 2020 were retrospectively analyzed.According to the different treatment methods,they were divided into observation and control groups,with 100 cases in each group;the observation group was treated with ultra pulse CO_(2) lattice laser,and the control group was treated with skin grinding.The t test was used to compare the ECCA scores and quality of life scores between the two groups before and after treatment.Theχ^(2) test was used to compare the clinical efficacy and adverse reactions of the two groups.Results The total effective rate of clinical curative effect in the observation group was 92.0%,which was significantly higher than 84.0%in the control group,and the difference was statistically significant(χ^(2)=5.125,P<0.05);before treatment,there was no statistically significant difference between the ECCA score and the quality of life score between the two groups(t=1.254,1.174;all P>0.05);after treatment,the ECCA score and quality of life score in the observation group were significantly higher than those of the control group,and the difference was statistically significant(t=5.412,5.326;all P<0.05);the incidence of adverse reactions in the observation group was 31.0%,significantly lower than 37.0%of the control group.The difference was statistically significant(χ^(2)=5.374,P<0.05).Conclusions Ultra pulse CO_(2) lattice laser to treat the acne atrophic scars has a better therapeutic effect and safety than skin grinding,and it has significant clinical value for reducing the rate of adverse reactions and improving the patients´quality.
