摘 要:弥散张量成像(DTI)是一种能够无创显示脑内白质纤维束结构的磁共振成像方式,在缺血性脑卒中、神经系统肿瘤等疾病的诊断及治疗中发挥重要作用。脑深部电刺激术(DBS)作为一种被广泛用于特发性震颤、帕金森病的有效治疗方法,通过植入脑内特定灰质核团的电极介导的电刺激发挥治疗作用,具体作用机制尚不清楚,但脑内白质纤维束参与了DBS的作用机制。近年来,利用DTI探究DBS治疗特发性震颤及帕金森病涉及的大脑结构,利用DTI辅助DBS定位特定的白质纤维束等研究逐渐引起了研究者的关注。将DTI应用于特发性震颤及帕金森病的DBS中对于阐释DBS的作用机制、选择并定位最佳的手术靶点等具有重要意义。Diffusion tensor imaging(DTI)is a magnetic resonance imaging method that can non-invasively display the structure of white matter fiber bundles in the brain,which has played an important role in the diagnosis and treatment of diseases such as ischemic stroke and nervous system tumors.Deep brain stimulation(DBS)is widely used as an effective treatment method for essential tremor and Parkinson′s disease.It uses electrical stimulation mediated by electrodes implanted in specific gray matter nuclei in the brain to perform the treatment.Although the specific mechanism of action is not yet clear,white matter fiber bundles in the brain are involved in the mechanism of DBS.In recent years,the use of DTI to explore the brain structures involved in the treatment of essential tremor and Parkinson′s disease with DBS,and the use of DTI to assist DBS to locate specific white matter fiber bundles have attracted the researchers′interest.The application of DTI to the DBS for essential tremor and Parkinson′s disease is of great significance for explaining the action mechanism of DBS,selecting and positioning the best surgical target,etc.
