摘  要:糖尿病心肌病是一种由糖尿病引起的独立于冠心病、高血压和结构性心脏病等明确病因的特异性心肌病。糖脂代谢紊乱可导致心肌细胞氧化应激、钙离子失调、炎症及微循环障碍,进而引起心肌细胞肥大、凋亡以及功能损害,最终导致心脏结构和功能异常而发生心力衰竭。随着糖尿病发病率的逐年升高,糖尿病心肌病已成为糖尿病患者死亡的主要原因。钠-葡萄糖协同转运蛋白-2(SGLT-2)抑制剂是近年来研发的新型降糖药,其在降低血糖的同时能有效保护糖尿病患者的心脏功能。虽然SGLT-2抑制剂可单独作用于心脏,但具体机制目前尚不明确。未来还需更多的研究进一步验证SGLT-2抑制剂在心脏的作用靶点,为糖尿病心肌病的治疗提供更可靠的依据。Diabetic cardiomyopathy is a specific cardiomyopathy,which is caused by diabetes and independent of coronary heart disease,hypertension and structural heart disease.The disorder of glucose and lipid metabolism can induce the oxidative stress,calcium ion imbalance,inflammation and microcirculatory disturbance of myocardial cells,which cause hypertrophy,apoptosis and functional damage of myocardial cells,leading to structural and functional abnormalities of the heart and finally resulting in heart failure.With the increasing incidence of diabetes year by year,diabetic cardiomyopathy has gradually become the main cause of death in diabetic patients.Sodium-glucose cotransporters 2(SGLT-2)inhibitor is a newly developed hypoglycemic drug in recent years which can effectively protect the heart function of diabetic patients while reducing blood glucose.At present,it is believed that SGLT-2 inhibitors can independently plays a role in the heart,but the specific mechanism remains unclear.In the future,more studies are needed to verify the target of SGLT-2 inhibitors in the heart for providing a more reliable basis for the treatment of diabetic cardiomyopathy.
