摘　　要：目的探讨二维应变成像技术评价健心颗粒对心力衰竭大鼠左心室功能的影响。方法40只SD大鼠,按随机数字表法分为假手术组、模型组、健心颗粒组和苯那普利组进行干预。应用缩窄腹主动脉法制备心力衰竭大鼠模型。对造模成功大鼠于给药前及给药12周行常规超声心动图检查;采用二维应变成像技术分析左心室心肌收缩期整体纵向应变(GLS)和周向应变(GCS)峰值。酶联免疫吸附试验法测定血浆血管紧张素Ⅱ(AngⅡ)、N末端前脑钠肽(NT-proBNP)水平;原位缺口末端标记检查左心室肌细胞凋亡;MASSON染色分析左心室肌纤维化。结果与假手术组比较,模型组左心室射血分数降低,左心室扩大,GLS和GCS应变值减低,血AngⅡ、NT-proBNP增高,差异有统计学意义(P<0.05)。健心颗粒或苯那普利治疗后,上述指标均改善。结论二维应变成像技术能定量评价心力衰竭大鼠左心室功能,为健心颗粒治疗心力衰竭提供了新的理论依据。Objective To explore the effect of Jianxin Granule on left ventricular function in rats with heart failure by using two-dimensional speckle tracking imaging(STI). Methods Forty SD rats were divided into sham operation group, model group, Jianxin Granule group, and benazepril group according to the random number table method for intervention. Heart failure was induced by abdominal aorta constriction in the model group and the drug intervention groups. Left ventricular function was detected by ultrasound before and 12 weeks after administration. The STI was used to analyze the global longitudinal strain(GLS)and global circumferential strain(GCS)of left ventricularduring systole.Plasma angiotensin Ⅱ(AngⅡ) and N-terminal pro brain natriuretic peptide(NT-proBNP) levels were measured by enzyme-linked immunosorbent assay(ELISA). Left ventricle myocyteapotosis and fibrosis were analyzed by in situ nickend labeling and Masson staining, respectively. Results The model group showed reduced left ventricular ejection fraction(EF), enlarged left ventricle, decreased GLS and GCS, higher plasma Ang Ⅱ and NT-ProBNP compared with the sham operation group. These differences were statistically significant(P <0.05). The above indicators were all improved after Jianxin Granule or benazepril treatment. Conclusion The STI can quantitatively evaluate left ventricle function in rats with heart failure, which provides a new theoretical basis for Jianxin Granule in treating heart failure.
