低温等离子腺样体消融术联合鼓膜穿刺治疗腺样体肥大合并分泌性中耳炎患儿的效果分析

摘　　要：目的:探讨低温等离子腺样体消融术联合鼓膜穿刺治疗腺样体肥大合并分泌性中耳炎(SOM)患儿的效果。方法:将某院2017年2月~2020年5月期间91例(106耳)腺样体肥大合并SOM患儿分组,按照计算机分组法分为对照组45例(51耳)和观察组46例(55耳),对照组给予低温等离子腺样体消融术治疗,观察组在对照组基础上给予鼓膜穿刺治疗,观察两组治疗效果、临床指标、免疫功能和术后并发症。结果:治疗后,观察组总有效率(91.30%)显著高于对照组(75.56%)(P<0.05);观察组中耳积液、鼓膜愈合、听力恢复正常、耳鸣消失时间均显著短于对照组(P<0.05);治疗后,观察组CD4^(+)、CD4^(+)/CD8^(+)显著高于对照组,CD8^(+)显著低于对照组(P<0.05)。结论:低温等离子腺样体消融术联合鼓膜穿刺可改善腺样体肥大合并SOM患儿临床症状和免疫功能,进而提高治疗效果且不会增加术后并发症。Objective:To investigate the effect of low temperature plasma adenoidectomy combined with tympanic membrane puncture in the treatment of children with adenoid hypertrophy and secretory otitis media(SOM).Methods:From February 2017 to may 2020,91 cases(106 ears)of children with adenoid hypertrophy and SOM were divided into control group(45 cases,51 ears,low temperature plasma adenoidectomy)and observation group(46 cases,55 ears,tympanic membrane puncture based on the control group)by computer grouping method.The treatment effect,clinical indexes,immune function and postoperative complications of the two groups were observed.Results:After treatment,the total effective rate of the observation group(91.30%)was significantly higher than that of the control group(75.56%)(P<0.05);The time of middle ear effusion,tympanic membrane healing,hearing recovery and tinnitus disappearance in the observation group were significantly shorter than those in the control group(P<0.05).After treatment,CD4^(+),CD4^(+)/CD8^(+)of the observation group were significantly higher than those of the control group,and CD8^(+)was significantly lower than that of the control group(P<0.05).Conclusion:Low temperature plasma adenoidectomy combined with tympanic membrane puncture can improve the clinical symptoms and immune function of children with adenoid hypertrophy combined with SOM,and improve the treatment effect without increasing postoperative complications.
