一次性包皮环切吻合器与包皮环扎术在包皮儿童治疗中的临床对比

摘　　要：目的:对比一次性包皮环切吻合器与包皮环扎术在包皮儿童治疗中的临床效果。方法:前瞻性选择2019年9月~2020年8月某院收治的90例包皮患儿作为研究对象,依据随机数字表法分为环扎术组和环切吻合器组各45例,环扎术组采用包皮环扎术治疗,环切吻合器组采用一次性包皮环切吻合器治疗。比较两组手术指标(术中出血量、手术时间、切口完全愈合时间)和术后1个月及术后3个月时美观性(美观满意度调查量表评分),并记录对比两组术后并发症发生情况。结果:环扎术组出血量低于环切吻合器组,手术时间短于环切吻合器组,切口完全愈合时间长于环切吻合器组,差异有统计学意义(P<0.05);两组术后3个月时的美观满意度调查量表评分均高于于术后1个月,且环切吻合器组术后1个月以及术后3个月时的美观满意度调查量表评分均高于环扎术组,差异有统计学意义(P<0.05);环切吻合器组术后并发症总发生率低于环扎术组,差异有统计学意义(P<0.05)。结论:包皮环扎术在缩短手术时间、降低出血量方面更有优势,而一次性包皮环切吻合器在缩短切口愈合时间,降低术后并发症发生率以及美观性方面更优。Objective:To compare the clinical effect of disposable circumcision stapler and circumcision in the treatment of children with foreskin.Methods:90 children with foreskin admitted to a hospital from September 2019 to August 2020 were prospectively selected as the research objects.According to the random number table method,they were divided into cerclage group and circumcision stapler group,with 45 cases in each group.The cerclage group was treated with circumcision,and the circumcision stapler group was treated with a disposable circumcision stapler.The surgical indicators(intraoperative blood loss,operation time,complete healing time of the incision),aesthetics at 1 month and 3 months after surgery(aesthetic satisfaction survey scale),and the incidence of postoperative complications in the two groups were recorded and compared.Results:The blood loss of the cerclage group was lower than that of the circumcision stapler group,the operation time was shorter than that of the circumcision stapler group,and the complete healing time of the incision was longer than that of the circumcision stapler group.The difference was statistically significant(P<0.05).The aesthetic satisfaction survey scores at the last 3 months were higher than those at 1 month after surgery,and the aesthetic satisfaction survey scores at 1 month after surgery and 3 months after surgery in the circumcision stapler group were both higher than those in the cerclage group,the difference was statistically significant(P<0.05).The total incidence of postoperative complications in the circumcision stapler group was lower than that of the cerclage group,and the difference was statistically significant(P<0.05).Conclusion:Circumcision has more advantages in shortening the operation time and reducing the amount of bleeding,and the disposable circumcision stapler is better in shortening the healing time of the incision,reducing the incidence of postoperative complications and aesthetics.
