人工全髋关节置换术治疗老年移位的头下型股骨颈骨折的中期疗效及生存质量

摘　　要：目的:分析人工全髋关节置换术治疗老年移位的头下型股骨颈骨折的中期疗效及生存质量。方法:选取2019年1月~2020年1月在某院入院并且确诊为头下型股骨径骨折的患者共130例,利用随机分组的方式将其分为对照组和观察组各65例,对照组使用人工股骨头置换术,观察组使用全髋关节置换术,观察两组患者的各项手术指标、髋关节功能的康复效果以及术后并发症等。结果:经过全髋关节置换术的实施,各项指标都明显优于人工股骨头置换术,观察组患者优良率为96.92%,对照组患者优良率为69.23%,观察组优良率明显高于对照组;全髋关节置换术患者并发症发生率为1.54%,采取人工股骨头置换术患者并发症发生率为13.85%,全髋关节置换术并发症发生率更低。结论:在老年移位的头下型股骨颈骨折治疗中,全髋关节置换术的治疗效果要优于人工股骨头置换术的治疗效果,不仅可以减少术后并发症发生率,还可以加快关节功能的恢复,有效提升了患者的生存质量,临床疗效比较满意。Objective:To analyze the mid-term efficacy and quality of life of artificial total hip arthroplasty in the treatment of elderly patients with displaced head-type femoral neck fractures.Methods:From January 2019 and January 2020,130 patients with displaced head-type femoral neck fractures treated in a hospital were selected and randomly divided into control group and observation group.The artificial femoral head replacement was used in the control group,and the artificial total hip arthroplasty was used in the observation group.The surgical indicators,the rehabilitation effect of hip function and postoperative complications in the two groups were observed.Results:After the implementation of total hip arthroplasty,all indicators were significantly better than those of artificial femoral head replacement.The excellent rate of patients in the observation group was 96.92%,the excellent rate of patients in the control group was 69.23%,and the excellent rate in the observation group was significantly higher than that in the control group.The incidence of complications in patients with artificial total hip arthroplasty was 1.54%,and the incidence of complications in patients with artificial femoral head replacement was 13.85%.The incidence of complications of artificial total hip arthroplasty was lower.Conclusion:In the treatment of elderly patients with displaced head-type femoral neck fractures,the effect of total hip arthroplasty is better than that of artificial femoral head replacement,which can not only reduce the incidence of postoperative complications,but also accelerate the recovery of joint function,and effectively improve the quality of survival of patients,with relatively satisfied clinical effect.
