不同栓塞剂栓塞子宫动脉对重症子宫腺肌症患者痛经疼痛程度、血清FSH水平的影响

摘　　要：目的探讨不同栓塞剂栓塞子宫动脉对重症子宫腺肌症患者痛经疼痛程度、血清卵泡刺激素(FSH)水平的影响。方法选取90例行子宫动脉栓塞术治疗重症子宫腺肌症患者,根据术中栓塞剂不同分为A组(27例)、B组(31例)、C组(32例)。术后1、3、6、12个月随访,观察比较三组痛经疼痛程度、月经量、子宫体积及血清FSH水平变化。结果术前,三组痛经疼痛程度评分、月经量、子宫体积及血清FSH水平比较,差异无统计学意义(P>0.05);术后1、3、6、12个月,B组、C组痛经疼痛程度评分、血清FSH水平均显著低于A组,月经量少于A组,子宫体积小于A组,差异有统计学意义(P<0.05);不同时间各组痛经疼痛程度评分、月经量、子宫体积、血清FSH水平组内比较,差异有统计学意义(P<0.05)。结论子宫动脉栓塞术治疗重症子宫腺肌症中PVA或海藻酸钠微球与明胶海绵颗粒联合使用效果优于单纯明胶海绵颗粒栓塞。Objective To investigate the effects of different embolization agents on dysmenorrhea pain and serum follicle stimulating hormone(FSH)level in patients with severe adenomyosis.Methods Ninety patients with severe adenomyosis underwent uterine artery embolization were divided into group A(27 cases),group B(31 cases)and group C(32 cases)according to different embolization agents.The degree of dysmenorrhea,menstrual volume,uterine volume and serum FSH level of the three groups were observed and compared at 1,3,6 and 12 months after operation.Results Before surgery,there were no significant differences in dysmenorrhea score,menstrual volume,uterine volume and serum FSH level among the three groups(P>0.05).At 1,3,6 and 12 months after operation,the score of dysmenorrhea and the level of serum FSH in groups B and C were significantly lower than that in group A,the amount of menstruation was less than that in group A,and the uterine volume was smaller than that in group A,with statistical significance(P<0.05).The scores of dysmenorrhea pain,menstrual volume,uterine volume and serum FSH level were compared among groups at different time,and the differences were statistically significant(P<0.05).Conclusion In the treatment of severe adenomyosis by uterine artery embolization,the effect of PVA or sodium alginate microspheres combined with gelatin sponge particles is better than that of gelatin sponge particles alone.
