不同激素治疗方案对围绝经期异常子宫出血的疗效研究

摘　　要：目的评价围绝经期异常子宫出血患者应用不同激素治疗方案的临床效果。方法选取100例围绝经期异常子宫出血患者,根据随机数字表法分为对照组(46例)与试验组(54例)。对照组采用醋酸甲羟孕酮治疗,试验组采用左炔诺孕酮治疗,比较两组临床疗效、治疗前后贫血指标、月经量、经期时间及子宫内膜厚度。结果试验组治疗总有效率为98.15%,明显高于对照组的84.78%,差异有统计学意义(P<0.05)。治疗前,两组患者血红蛋白、红细胞比容比较,差异无统计学意义(P>0.05);治疗后,两组血红蛋白、红细胞比容均高于治疗前,且试验组明显高于对照组,差异具有统计学意义(P<0.05)。治疗前,两组患者月经量、经期时间和子宫内膜厚度比较,差异无统计学意义(P>0.05);治疗后,两组月经量少于治疗前,经期时间短于治疗前,子宫内膜厚度薄于治疗前,试验组月经量为(24.18±9.82)ml,经期时间(4.02±0.33)d,子宫内膜厚度(5.28±1.22)mm,对照组月经量为(40.51±11.52)ml,经期时间(6.55±0.57)d,子宫内膜厚度(8.52±1.51)mm,试验组月经量少于对照组,经期时间短于对照组,子宫内膜厚度薄于对照组,差异具有统计学意义(P<0.05)。结论围绝经期异常子宫出血患者采用激素治疗可以取得显著的成效,但与醋酸甲经孕酮相比,左诀诺孕酮的效果更佳,值得临床推广与应用。Objective To evaluate the clinical effect of different hormone therapy in perimenopausal patients with abnormal uterine bleeding.Methods A total of 100 patients with abnormal uterine bleeding during perimenopause were selected and divided into control group(46 cases)and experimental group(54 cases)according to random number table method.The control group was treated with medroxyprogesterone acetate,and the experimental group was treated with levonorgestrel.The clinical efficacy,anemia indexes,menstrual volume,menstrual duration and endometrial thickness of the two groups were compared before and after treatment.Results The total effective rate of the experimental group was 98.15%,which was significantly higher than that of the control group(84.78%),the difference was statistically significant(P<0.05).Before treatment,there was no statistical significance in the comparison of hemoglobin and erythrocyte volume between 2 groups(P>0.05).After treatment,hemoglobin and RBC specific volume in both groups were higher than before treatment,and the experimental group was significantly higher than the control group,the difference was statistically significant(P<0.05).Before treatment,there were no significant differences in menstrual volume,menstrual time and endometrial thickness between 2 groups(P>0.05).After treatment,the menstrual volume,menstrual duration and endometrial thickness of the two groups were less than before treatment,shorter than before treatment,and thinner than before treatment.Menstrual volume of the test group was(24.18±9.82)ml,menstrual duration was(4.02±0.33)d,endometrial thickness was(5.28±1.22)mm,and menstrual volume of the control group was(40.51±11.52)ml.Menstrual duration was(6.55±0.57)d and endometrial thickness was(8.52±1.51)mm.Menstrual volume,menstrual duration and endometrial thickness in the experimental group were less than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Hormone therapy in perimenopausal patients with abnormal uterine
