摘　　要：目的探讨3D腹腔镜下肝门空肠Roux-y吻合术治疗小儿Ⅲ型胆道闭锁的手术配合及护理。方法回顾性分析2017年12月-2019年12月Ⅲ型胆道闭锁患儿30例作为对象,所有患儿均在3D腹腔镜下行肝门空肠Roux-y吻合术治疗,随机数字表法分为对照组和观察组,各15例。对照组采用常规护理,观察组在对照组基础上联合护理配合,7 d护理后对患者效果进行评估,比较2组并发症发生率、知晓率、满意度及家属自我效能。结果观察组护理后7 d并发症发生率、家属知晓率、护理满意度、护理后7 d家属自我效能评分均优于对照组(P<0.05)。结论加强小儿Ⅲ型胆道闭锁患者3D腹腔镜下肝门空肠Roux-y吻合术治疗时护理配合能降低术后并发症发生率,提高窒息率/满意度,亦可提供家属自我效能,值得推广应用。Objective To explore the effect of the surgical nursing cooperation in the treatment of Type III biliary atresia in children with 3D laparoscopic Roux-y hepaticojejunostomy.Methods A total of 30 children with Type III biliary atresia from December 2017 to December 2019 were selected as the research subjects and retrospectively analyzed,who were all treated with 3D laparoscopic Roux-y hepaticojejunostomy.They were randomly divided into control group and observation group,each with 15 cases.The control group was given routine nursing care,and the observation group was given nursing cooperation on the basis of the control group.After 7 days,the patients’effect was evaluated,and the complication rate,awareness rate,satisfaction rate and family self-efficacy of the two groups were compared.Results The incidence of complications,awareness rate of family members,nursing satisfaction and self-efficacy score of family members in the observation group were better than those in the control group(P<0.05).Conclusion Strengthening the nursing cooperation in the treatment of pediatric Type III biliary atresia with 3D laparoscopic Roux-y hepaticojejunostomy can reduce the incidence of postoperative complications,improve the patients’asphyxia rate and satisfaction,and also provide the family self-efficacy,which is worthy of promotion and application.
