摘　　要：目的:观察糖尿病酮症患者成纤维细胞生长因子21(FGF21)的变化。方法:选择内分泌科住院的糖尿病酮症患者21例为A组,其中1型糖尿病合并酮症10例,2型糖尿病合并酮症11例;同期住院的非酮症的糖尿病患者24例为B组,其中1型糖尿病10例,2型糖尿病14例;同期体检的非糖尿病患者24例为C组。均空腹10 h,于次日清晨抽取肘静脉血,检测血甘油三酯(TG)、低密度脂蛋白(LDL)、血糖(FBG)、C肽、糖化血红蛋白(HbA1C)、FGF21和胰高血糖素(GC)。结果:A组FBG,HbA1C,FGF21水平明显高于B组(P<0.05),其中1型糖尿病组HbA1C和FGF21水平明显高于2型糖尿病组(P<0.05)。B组FBG和HbA1C明显高于C组(P<0.05)。B组和C组FGF21比较,差异无统计学意义(P>0.05)。三组GC水平比较,差异无统计学意义(P>0.05)。结论:糖尿病酮症患者的FGF21明显增高,其中1型糖尿病酮症组明显高于2型糖尿病酮症组。Objective:To study the changes of circulating fibroblast growth factor 21(FGF21)in diabetic patients with ketosis.Methods:All 21 patients with diabetic ketosis(group A,including 10 type 1 diabetic ketosis and 11 type 2 diabetic ketosis),24 diabetic patients without ketosis(group B,including 10 type 1 diabetic patients and 14 type 2 diabetic patients)and 24 normal controls(group C)were studied.Fasting blood glucose(FBG),triglyceride(TG),low density lipoprotein(LDL),C peptide,glycosylated hemoglobin A1C(HbA1c),FGF21,and glucagon(GC)were measured after 10 hours of overnight fasting.Results:The levels of FBG,HbA1C and FGF21 in group A were increased significantly compared with group B(P<0.05).The levels of HbA1C and FGF21 in type 1 diabetic patients were significantly increased than that in type 2 diabetic patients(P<0.05).The levels of FBG and HbA1C in group B were increased significantly compared with group C(P<0.05),but there were no significant differences of FGF21 between group B and C(P>0.05).There are no significant differences of GC among the three groups(P>0.05).Conclusion:The levels of FGF21 in diabetic patients with ketosis were increased significantly.The levels of FGF21 in type 1 diabetic patients with ketosis were higher than that in type 2 diabetic ketosis.
