摘 要:目的:探讨冻融周期与新鲜周期胚胎移植对行体外受精-胚胎移植(IVF-ET)助孕的多囊卵巢综合征(PCOS)患者临床妊娠结局的影响。方法:选取2017年2月—2019年3月拟行IVF-ET助孕患者964例,根据胚胎类型不同分为冻融周期组(n=492)与新鲜周期组(n=472)。新鲜周期组取新鲜胚胎种植,冻融周期组胚胎冷冻后行冻融胚胎移植。对比两组促排卵情况、种植率、妊娠情况、新生儿出生体质量和低体重出生儿比例。结果:新鲜周期组促性腺激素(Gn)总用量、获卵数、人绒毛膜促性腺激素(hCG)日卵泡数、种植率、临床妊娠率低于冻融周期组,hCG日孕激素(P)、雌激素(E2)、异位妊娠率高于冻融周期组,差异有统计学意义(P<0.05)。两组Gn初始计量、早产率、多胎率、自然流产率、低体重出生儿比例和新生儿出生体质量对比,差异无统计学意义(P>0.05)。结论:与新鲜胚胎种植相比,以冻融胚胎移植行IVF-ET助孕可提高种植率和临床妊娠率,降低异位妊娠,获得更好的妊娠结局。Objective:To investigate the effect of freeze-thaw cycle and fresh cycle embryo transfer on the clinical pregnancy outcome of patients with polycystic ovary syndrome(PCOS)undergoing in vitro fertilization-embryo transfer(IVF-ET).Methods:A total of 964 patients planned to undergo IVF-ET in hospital from February 2017 to March 2019 were selected and divided into freeze-thaw cycle group(n=492)and fresh cycle group(n=472)according to different embryo types.Fresh embryos were planted in the fresh cycle group,and freeze-thaw embryos were transplanted after freezing the embryos in the freeze-thaw cycle group.The two groups were compared for ovulation induction,implantation rate,pregnancy,newborn birth weight and the proportion of low birth weight.Results:The total dosage of gonadotropin(Gn),the number of eggs obtained,the number of human chorionic gonadotropin(hCG)daily follicles,implantation rate,and clinical pregnancy rate in the fresh cycle group were lower than those in the freeze-thaw cycle group,and hCG daily progesterone(P),estrogen(E2)and ectopic pregnancy was higher than that of the freeze-thaw cycle group,and the difference was statistically significant(P<0.05).There was no significant difference in Gn initial measurement,premature birth rate,multiple birth rate,spontaneous abortion rate,low birth weight ratio and birth weight of newborns between the two groups(P>0.05).Conclusion:Compared with fresh embryo implantation,IVF-ET assisted pregnancy with freeze-thaw embryo transfer can increase implantation rate,clinical pregnancy rate,reduce ectopic pregnancy,and obtain better pregnancy outcome.
