摘　　要：目的:探讨炎性指标对儿童支原体肺炎(MPP)的早期预测作用。方法:对已行支气管镜检查及治疗的246例MPP患儿的临床资料进行回顾性分析。比较MPP支气管镜下阻塞组(148例)和支气管镜下无阻塞组(98例)患儿的临床资料以及年龄、性别、血沉、乳酸脱氢酶(LDH)、D-二聚体、白细胞计数、中性粒细胞百分比、C-反应蛋白(CRP)、降钙素原(PCT)等指标,应用ROC曲线评估预测支气管镜下阻塞的特异性指标。结果:MPP患儿支气管镜下阻塞多见于≥6岁儿童。两组D-二聚体、LDH比较,差异有统计学意义(P<0.05)。ROC曲线分析显示D-二聚体及LDH对预测镜下阻塞的曲线面积均在0.8以上,且LDH≥335 U/L可作为MPP镜下阻塞的预测指标。结论:LDH和D-二聚体可作为MPP支气管镜下阻塞的预测指标。Objective:To explore the early predictive effect of inflammatory indicators on childhood mycoplasma pneumonia(MPP).Methods:The clinical data of 246 children with MPP who had undergone bronchoscopy and treatment were retrospectively analyzed.To compare the clinical data of children with MPP bronchoscopy obstruction group(148 cases)and bronchoscopy non-obstruction group(98 cases),as well as different age,gender,erythrocyte sedimentation rate,lactate dehydrogenase(LDH),D-dimer,white blood cell count,percentage of neutrophils,C-reactive protein(CRP),procalcitonin(PCT)and other laboratory indicators,use ROC curve to evaluate specific indicators for predicting obstruction under bronchoscopy.Results:Bronchoscopy obstruction in children with MPP is more common in children over 6 years old.And the difference between D-dimer and LDH was statistically significant between the two groups(P<0.05).ROC curve analysis showed that the curve area of D-dimer and LDH for predicting obstruction under microscope was above 0.8,and LDH above 335 U/L can be used as a predictive index for predicting obstruction under MPP.Conclusion:LDH and D-dimer can be used as predictors of obstruction under MPP bronchoscopy.
