从体质学说探讨胆石症的防治

摘　　要：胆石症患者常见体质类型为湿热质、痰湿质、气郁质及阴虚质,临床常见证型为湿热蕴结证、痰湿阻滞证、肝郁气滞证及肝阴不足证。证型与胆石症常见发病体质有一定相关性,根据体质辨证制定治疗方案是治疗的关键。属湿热质者可选大柴胡汤加减以清热利湿、疏利肝胆;痰湿质者用藿朴夏苓汤加减以燥湿化痰、疏利肝胆;属气郁质者用柴胡疏肝散加减以行气解郁、疏利肝胆;属阴虚质者用一贯煎加减以滋阴益气、疏利肝胆。此外,在体质调理的同时应尤其重视饮食规律、起居有常及情志舒畅。The common constitution types of cholelithiasis patients include dampness-heat constitution,phlegm-dampness constitution,qi stagnation constitution,and yin deficiency constitution.The common clinical syndrome types include dampness-heat accumulation syndrome,phlegm-dampness blockage syndrome,liver depression and qi stagnation syndrome,and liver yin deficiency syndrome.There is a certain correlation between syndrome type and common constitution of cholelithiasis.It is the key to make the treatment plan according to the constitution differentiation.For dampness-heat constitution,Modified Major Bupleurum Decoction can be used to clear heat,remove dampness,soothe liver and gallbladder.For those with phlegm-dampness constitution,Modified Agastache and Magnolia and Pinellia and Poria Decoction can be used to dry dampness,resolve phlegm,soothe liver and gallbladder.For qi stagnation constitution,Modified Bupleurum Liver-Soothing Powder to move qi stagnation and soothe liver and gallbladder.For those with yin deficiency constitution,Modified Effective Integration Decoction is used to nourish yin,replenish qi,soothe liver and gallbladder.In addition,in the physical conditioning at the same time,it is necessary to pay special attention to diet,daily life and emotional comfort.
