自制“活血通络膏”联合玻璃酸钠治疗风寒湿痹型膝痹病(膝骨关节炎)的临床疗效

摘　　要：目的:观察自制“活血通络膏”外用联合膝关节腔内注射玻璃酸钠治疗风寒湿痹型膝骨关节炎的临床疗效和安全性。方法:将膝骨关节炎患者60例按就诊先后顺序随机分为观察组、对照组各30例。观察组行自制“活血通络膏”联合膝关节腔内注射玻璃酸钠治疗,对照组行膝关节腔内注射玻璃酸钠治疗,两组均连续治疗5周为1个疗程。观察治疗前后两组患者VAS评分、WOMAC骨关节炎指数评分、中医证候积分变化情况及临床疗效与不良反应发生情况。结果:总有效率观察组高于对照组(P<O.05),VAS评分、WOMAC骨关节炎指数评分、中医证候积分观察组低于对照组(P<O.05);ALT、AST、BUN及SCr两组比较差异无统计学意义(P>O.05)。结论:“活血通络膏”外敷与膝关节腔内注射玻璃酸钠能通络止痛,祛风除湿,温里散寒,提高风寒湿痹型膝痹病的临床疗效,且安全有效。Objective:To observe clinical effects and the safety of selfmade“blood-activating collaterals dredging ointment”for external application combined with intra articular injection of sodium hyaluronate in treating knee osteoarthritis of wind-cold-damp arthralgia pattern.Methods:Sixty patients were randomized into the observation group and the control group according to the visiting order,30 cases each group.The observation group were treated by selfmade“blood-activating collaterals-dredging ointment”for external application combined with intra articular injection of sodium hyaluronate,and the cotnrol group by intra articular injection of sodium hyaluronate,both groups were treated for five consecutive weeks as one course of the treatment.To observe VAS scores,WOMAC,TCM syndrome integrals,clinical effects and the incidence of adverse reaction before and after treating between both groups.Results:Total effective rate of the observation group was higher than that of the control group(P<0.05).The observation group was lower than the control group in VAS scores,WOMAS and TCM syndrome integrals(P<0.05);the difference had no statistical meaning in ALT,AST,BUN and SCr between both groups(P>0.05).Conclusion:Selfmade“blood-activating collaterals-dredging ointment”for external application combined with intra articular injection of sodium hyaluronate in treating knee osteoarthritis of wind-cold-damp arthralgia pattern could unblock collaterals and relieve pain,dispel wind and remove dampness,warm the interior and dispel cold,which is safe and effective.
