针刺治疗脑卒中后抑郁症腧穴配伍规律文献研究

摘　　要：目的:挖掘脑卒中后抑郁症现代针灸治疗的用穴和腧穴配伍规律。方法:检索中国知网(CNKI)、万方数据库(WF)、维普期刊数据库(VIP)及PubMed数据库中截至2019年3月1日针刺治疗抑郁症的临床研究文献,按照预先制定的纳入、排除标准进行筛选,建立针刺治疗脑卒中后抑郁症处方数据库,采用IBM SPSS MODLDER和Gephi软件对数据库进行关联规则和复杂网络分析。结果:共纳入317个临床对照试验。运用的腧穴频次最高为百会穴,其次为内关、太冲、印堂、神门等,经外奇穴中四神聪运用较多;以局远配穴为主。百会(DU20)、神庭(DU24)、水沟(DU26)、四神聪(EX-HN1)、三阴交(SP6)、合谷(LI4)、足三里(ST36)、印堂(EX-HN3)、内关(PC6)、神门(HT7)、太冲(LR3)和风池(GB20)为核心节点;百会和印堂,百会和神庭,百会和四神聪,百会、内关和神门,百会、三阴交和内关,百会、内关和太冲,太冲和合谷之间具有强相关性。结论:针刺治疗脑卒中后抑郁症配穴多于单穴,以局远配穴为主,核心节点中具有强相关性的穴组可单独或联合应用。Objective:To excavate the laws of acupoint selection and acupoints compatibility of modern acupuncture in the treatment of post-stroke depression(PSD).Methods:Clinical research literature of acupuncture in treating depression were search from CNKI,WF,VIP,PubMed database and others by March 1st,2019,the prescription database for acupunture treatment of PSD was established according to the pre-set inclusion and exclusion criteria,IBM SPSS MODLDER and Gephi software were adopted to analyze the association rules and complex network of the database.Results:A total of 317 clinical controlled trials were included.The acupoint with the highest frequency was Baihui(GV20,DU20),next were Neiguan(PC6),Taichong(LR3),Yintang(EX-HN3),Shenmen(HT7),and others,among extra meridian acupoints,Sishencong(EX-HN1)was applied more in local distant matching.Baihui(GV20,DU20),Shenting(DU24),Shuigou(DU26),Sishencong(EX-HN1),Sanyinjiao(SP6),Hegu(LI4),Zusanli(ST36),Yintang(EX-HN3),Neiguan(PC6),Shenmen(HT7),Taichong(LR3)and Fengchi(GB20)were chosen as the core nodes;Baihui and Yintang,Baihui and Shenting,Baihui and Sishencong,Baihui,Neiguan and Shenmen,Baihui,Sanyinjiao and Neiguan,Baihui,Neiguan and Taichong,Taichong and Hegu own strong correlations.Conclusion:Matching points for acupuncture therapy of PSD are more than single acupoint in local distant matching,and acupoint group with the strong correlation among the core nodes can be used alone or in combination.
