清热凉血法联合加味乌藜消白汤治疗白癜风临床研究

摘　　要：目的:探究清热凉血法联合加味乌藜消白汤对白癜风患者硫酸脱氢表雄酮(DHEA-S)、摄食抑制因子(NSF-1)、甲状腺球蛋白抗体(Anti-TG)水平的影响。方法:选取白癜风患者146例,按随机数字表法分为加味乌藜消白汤治疗的单药组及清热凉血法联合加味乌藜消白汤治疗的联合组。比较两组患者中医症候评分,对炎性因子[肿瘤坏死因子-α(TNF-α)、白介素(IL-6、IL-33)]水平及DHEA-S、NSF-1、Anti-TG水平检测,分析临床疗效。结果:治疗后,联合组主症评分、中医症候总分均低于单药组(P<0.05)。治疗后,两组血清TNF-α、IL-6、IL-33、NSF-1、Anti-TG水平及HAMD、HAMA评分均降低,DHEA-S水平升高,且联合组血清TNF-α、IL-6、IL-33、NSF-1、Anti-TG水平及HAMD、HAMA评分均低于单药组,DHEA-S水平高于单药组(P<0.05);联合组治疗有效率高于单药组(P<0.05)。结论:采用清热凉血法联合加味乌藜消白汤对白癜风患者进行治疗,患者中医症候评分降低,有效减轻炎症反应,提高自身免疫能力,改善心理状态。Objective:In this study,we investigated the effects of Qingre Liangxue method combined with modified Wuli Xiaobai decoction on the expression of dehydroepiandrosterone sulfate(DHEA-S),food inhibitory factor(NSF-1),thyroglobulin antibody in vitiligo patients,effect of anti TG levels.Methods:A total of 146 patients with vitiligo were selected.The patients were divided into the single drug group treated with modified Wuli Xiaobai decoction and the combined group treated with Qingre Liangxue method combined with modified Wuli Xiaobai decoction.The scores of TCM symptoms were compared between the two groups.The levels of TNF-α,IL-6,IL-33 and DHEA-S,NSF-1 and anti-TG levels were detected to analyze the clinical efficacy.Results:After treatment,the TCM syndrome master score and TCM syndrome total score in the combination group were lower than those in the monotherapy group(P<0.05).After treatment,serum TNF-α,IL-6,IL-33,NSF-1,anti-TG and the HAMD and HAMA scores were decreased,the DHEA-S level was increased,and serum TNF-αwas increased in the combination group,the levels of IL-6,IL-33,NSF-1,and anti-TG as well as HAMD and HAMA scores were lower and the level of DHEA-S was higher in the combination than in the monotherapy group(P<0.05).The effective rate of treatment was higher in the combination group than in the monotherapy group(P<0.05).Conclusion:Using the method of Qingre Liangxue method combined with modified Wuli Xiaobai decoction to treat vitiligo patients,the patients’TCM syndrome scores were reduced,which effectively alleviated inflammation,improved autoimmune capacity,and improved psychological status.
