摘 要:目的检测突发性耳聋患者血清微小RNA-431(miR-431)和Eya4(Eye absent 4)的表达情况,并探讨其表达水平与突发性耳聋患者耳蜗功能的相关性。方法选取2018年11月至2020年1月耳鼻喉科收治的突发性耳聋患者65例为突发性耳聋组。同期选取进行常规体检的健康人群70例为对照组。采用实时荧光定量PCR法(qRT-PCR)检测2组血清miR-431、Eya4表达水平;通过研究对象工作特征曲线(ROC)评估血清miR-431、Eya4表达水平在突发性耳聋中的诊断价值;分析血清miR-431、Eya4高低表达组患者耳蜗功能;并对影响突发性耳聋患者耳蜗功能的相关因素进行Logistic回归分析。结果突发性耳聋组血清miR-431表达水平显著高于对照组(P<0.05);血清Eya4表达水平显著低于对照组(P<0.05)。ROC结果显示,血清miR-431、Eya4诊断突发性耳聋的AUC分别为0.795、0.746,截断值分别为1.234、0.990,此时对应灵敏度分别为72.3%、87.7%,特异性分别为80.0%、57.1%。血清miR-431、Eya4联合检测突发性耳聋的AUC为0.811,此时对应灵敏度和特异性分别为73.8%、78.6%。高miR-431组患者耳蜗功能异常发生率明显高于低miR-431组(P<0.05);高Eya4组患者耳蜗功能异常发生率显著低于低Eya4组(P<0.05)。Logistic回归分析显示,血清miR-431高表达、Eya4低表达是影响突发性耳聋患者发生认知障碍的独立危险因素。结论miR-431在突发性耳聋患者血清中的表达水平明显升高,Eya4表达水平明显降低,两者对突发性耳聋具有一定诊断价值,且其表达水平与患者耳蜗功能异常密切相关,提示血清miR-431、Eya4可作为评估突发性耳聋患者病情及耳蜗功能的潜在生物学指标。Objective To investigate the expressions of microRNA-431(miR-431)and Eye absent 4(Eya4)in serum of patients with sudden deafness,and to explore the correlation between the expression levels and cochlear function of patients.Methods A total of 65 patients with sudden deafness who were treated in our hospital from November 2018 to January 2020 were enrolled in as observation group,and the other 70 healthy subjects were enrolled as control group.The expression levels of miR-431 and Eya4 were detected by real-time fluorescent quantitative PCR(qRT-PCR),and the diagnostic value of serum miR-431 and Eya4 expression levels for sudden deafness was evaluated by receiver operating characteristic curve(ROC).In addition,Logistic regression analysis was performed to analyze the related factors affecting the cochlear function of patients.Results The expression levels of miR-431 in observation group were significantly higher than those in control group(P<0.05),however,the expression levels of serum Eya4 in observation group were significantly lower than those in control group(P<0.05).The results of ROC showed that the AUC of serum miR-431 and Eya4 in diagnosis of sudden deafness was 0.795 and 0.746,respectively,and the cut off value was 1.234 and 0.990,respectively,moreover,the corresponding sensitivity was 72.3%and 87.7%,and the specificity was 80.0%and 57.1%,respectively.The AUC of sudden deafness detected by serum miR-431 and Eya4 was 0.811,and the corresponding sensitivity and specificity were 73.8%and 78.6%,respectively.The incidence of abnormal cochlear function in high miR-431 group was significantly higher than that in low miR-431 group(P<0.05),and the incidence of abnormal cochlear function in high Eya4 group was significantly lower than that in low Eya4 group(P<0.05).In addition,Logistic regression analysis showed that high expression of miR-431 and low expression of Eya4 were independent risk factors for cognitive impairment in patients with sudden deafness.Conclusion The serum levels of miR-431 in patients with sudde
