重症胰腺炎患者在ICU行CRRT治疗的临床效果及其护理

摘　　要：目的探讨重症胰腺炎患者在ICU行CRRT治疗的临床效果及其护理。方法回顾性选取2016年9月至2020年9月本院ICU重症胰腺炎患者72例,依据治疗与护理方法分为综合护理组(n=36)与常规护理组(n=36),分别实施CRRT治疗与综合护理、常规治疗与常规护理,统计分析两组患者的生命体征、血气分析、血清AMY水平、肝肾功能、不良反应发生情况、死亡情况。结果综合护理组患者护理后较护理前的T、R、P、MAP、CVP降低幅度均显著高于常规护理组(P <0.05)。综合护理组患者护理后较护理前的PaO_(2)、PaO_(2)/FiO_(2)升高幅度、血清AMY水平降低幅度均显著高于常规护理组(P <0.05)。综合护理组患者护理后较护理前的SCr、BUN、AST、ALT、ALB、TBIL水平降低幅度均显著高于常规护理组(P <0.05)。综合护理组患者的不良反应发生率、病死率均显著低于常规护理组(P <0.05)。结论重症胰腺炎患者在ICU行CRRT治疗的临床效果较常规治疗好,综合护理较常规护理更能有效巩固其临床效果。Objective To investigate the clinical effect and nursing care of patients with severe pancreatitis undergoing CRRT in ICU.Methods Seventytwo cases of patients with severe pancreatitis in ICU of our hospital from June 2018 to June 2020 were retrospectively selected and divided into CRRT treatment and comprehensive nursing group(comprehensive nursing group,n=36)and routine treatment and routine nursing group(routine nursing group,n=36)according to the nursing methods.The vital signs,blood gas analysis,serum Amy levels,liver and kidney functions,adverse reactions and deaths of the two groups were statistically analyzed.Results The decreases of T,R,P,MAP and CVP of the comprehensive nursing group were significantly higher than the routine nursing group(P<0.05).The increases of PaO_(2),PaO_(2)/FiO_(2) and decrease of serum AMY levels of the comprehensive nursing group were significantly higher than the routine nursing group(P<0.05).The decreases of levels of SCr,BUN,AST,ALT,ALB and TBIL of the comprehensive nursing group were significantly lower than the routine nursing group(P<0.05).The incidence of adverse reactions and mortality of the comprehensive nursing group were significantly lower than the routine nursing group(P<0.05).Conclusion The clinical effect of CRRT in ICU for patients with severe pancreatitis is better than routine treatment,and comprehensive nursing can more effectively consolidate the clinical effect than routine nursing.
