腹腔镜下经胆囊管途径与胆总管切开治疗胆总管结石的对比研究

摘　　要：目的:探讨腹腔镜下经胆囊管途径治疗胆总管结石的可行性、安全性。方法:回顾分析2015年1月至2018年1月为135例胆囊结石合并胆总管结石患者行腹腔镜手术的临床资料,其中65例行腹腔镜经胆囊管胆总管探查取石术(LTCBDE);70例行腹腔镜胆总管探查取石术(LCBDE)。对比两组患者一般情况、手术时间、术中出血量、腹腔引流时间、住院时间、住院费用、并发症发生率。结果:两组患者一般情况、术后并发症发生率差异无统计学意义(P>0.05),LTCBDE组术后住院时间、术后腹腔引流时间、住院费用、手术时间优于LCBDE组(P<0.05)。结论:LTCBDE可避免术后T管引流,提高生活质量,缩短住院时间,具有经济优势。Objective:To investigate the feasibility and safety of laparoscopic transcystic common bile duct exploration(LTCBDE)in the treatment of choledocholithiasis.Methods:The clinical data of 135 patients,who suffered from cholecystolithiasis and choledocholithiasis and underwent laparoscopic surgery from Jan.2015 to Jan.2018,were retrospectively analyzed.Among them,65 patients underwent LTCBDE and 70 patients underwent laparoscopic common bile duct exploration(LCBDE).The general condition,operative time,intraoperative bleeding volume,abdominal drainage time,hospital stay,hospitalization cost and complication incidence of the two groups were compared.Results:There was no statistically significant difference between the two groups in general condition and the incidence of postoperative complications(P>0.05).The postoperative hospital stay,abdominal drainage time,hospitalization cost and operation time of LTCBDE group were better than those of LCBDE group(P<0.05).Conclusions:LTCBDE can avoid T tube drainage,improve quality of life,shorten hospital stay,and has economic advantage.
