口服双歧杆菌三联活菌胶囊治疗血液透析患者蛋白质能量消耗的效果观察

摘　　要：目的探讨双歧杆菌三联活菌胶囊对维持性血液透析患者蛋白质能量消耗(PEW)的影响。方法选取2018年5—8月在我院肾内科接受治疗的维持性血液透析(HD)且符合PEW诊断的患者85例,随机分为治疗组(n=43)和对照组(n=42)。对照组给予基础治疗,包括规律透析治疗、调节钙磷代谢、纠正贫血、降血压、控制血糖等,治疗组在常规治疗基础上服用双歧杆菌三联活菌胶囊。观察两组治疗前后三角肌皮褶厚度(TSF)、中臂围(MAC)、中臂肌围(MAMC)、体重指数(BMI)等体表测量参数,改良定量主观整体营养评估(MQSGA)评分,血红蛋白(Hb)、前白蛋白(PA)、血清白蛋白(ALB)、总铁结合力(TIBC)、胆固醇(TC)、三酰甘油(TG)、血肌酐等实验室指标的变化,并比较两组疗效。结果治疗3个月后,治疗组治疗后MAMC、BMI、PA、TC、TIBC、MAC、TSF高于治疗前,MQSGA评分低于治疗前,差异有统计学意义(P<0.05)。对照组治疗后PA、TIBC、TSF低于治疗前,差异有统计学意义(P<0.05)。治疗后治疗组MQSGA评分低于对照组,BMI、TIBC、TC、MAC、TSF、MAMC高于对照组,差异有统计学意义(P<0.05)。两组ALB、HB、TG、血肌酐指标比较,差异无统计学意义(P>0.05)。治疗组治疗总有效率为76.74%,高于对照组的38.10%,差异有统计学意义(P<0.05)。结论口服双歧杆菌三联活菌胶囊能够改善HD患者营养评分及体表测量指标,对PEW治疗有效。Objective To investigate the effect of bifidobacterium triple viable capsules on protein-energy wasting(PEW)in maintenance hemodialysis patients.Methods A total of 85 patients who were treated with maintenance hemodialysis(HD)in the nephrology department of our hospital from May to August 2018 and met the diagnosis of PEW were selected and randomly divided into the treatment group(n=43)and the control group(n=42).The control group was given basic treatment,including regular dialysis treatment,regulation of calcium and phosphorus metabolism,correction of anemia,blood pressure lowering,and blood glucose control,etc.The treatment group was administered bifidobacterium triple viable capsules on the basis of conventional treatment.Some parameters were observed before and after treatment in the two groups,such as triceps skinfold thickness(TSF),mid-arm circumference(MAC),mid-arm muscle circumference(MAMC),body mass index(BMI)and other body surface measurement parameters,modified quantitative subjective global assessment(MQSGA)score of nutrition,hemoglobin(Hb),prealbumin(PA),serum albumin(ALB),total iron binding capacity(TIBC),cholesterol(TC),triglyceride(TG),blood creatinine and other laboratory indicators,and compare the efficacy of the two groups.Results After 3 months of treatment,MAMC,BMI,PA,TC,TIBC,MAC,and TSF were higher and MQSGA scores were lower in the treatment group than those before treatment,with statistically significant differences(P<0.05).PA,TIBC,TSF in the control group after treatment were lower than those before treatment,with statistically significant differences(P<0.05).MQSGA score in the treatment group after treatment was lower than that in the control group,and BMI,TIBC,TC,MAC,TSF,and MAMC were higher than those in the control group,with statistically significant differences(P<0.05).There were no statistically significant differences in the comparison of ALB,HB,TG,and blood creatinine indexes between the two groups(P>0.05).The total effective rate of treatment in the treatment group was76.74%,wh
