摘 要:目的:探讨脾氨肽在儿童呼吸道反复感染(RRTI)不同类型中的效果。方法:采用前瞻性随机试验方法,选择2019年9月至2020年9月萍乡市人民医院收治的80例RRTI患儿,按照发病部位分为反复上呼吸道感染组(上感组,n=30)、反复支气管炎组(支气管组,n=28)和反复肺炎组(肺炎组,n=22)。3组同时给予脾氨肽辅助治疗。治疗3个月后,比较患儿外周血免疫球蛋白水平、淋巴细胞亚群和白介素-4(IL-4)的水平。结果:肺炎组治疗前免疫球蛋白G(IgG)水平低于上感组、支气管组(P<0.05);治疗3个月后,三组IgG、CD4^(+)、CD4^(+)/CD8^(+)水平均高于治疗前,CD8^(+)水平均低于治疗前(P<0.05);肺炎组治疗前IL-4水平高于上感组、支气管组(P<0.05);治疗3个月后,三组IL-4水平均低于治疗前(P<0.05)。结论:脾氨肽治疗儿童RRTI可改善患儿免疫学指标,提高机体抵抗力,且在反复肺炎患儿中效果更明显,值得临床运用。Objective:To investigate the effect of spleen aminopeptide on different types of recurrent respiratory tract infection in children.Methods:A prospective randomized trial method was conducted to select 80 children with RRTI admitted to Pingxiang People's Hospital from September 2019 to September 2020,the patients were divided into a recurrent upper respiratory tract infection group(upper respiratory tract infection group,n=30),a recurrent bronchitis group(bronchitis group,n=28)and a recurrent pneumonia group(pneumonia group,n=22)according to the location of onset.The 3 groups were given splenic amino peptide adjuvant therapy at the same time.3 months after treatment,the levels of peripheral blood immunoglobulin,lymphocyte subsets and interleukin-4(IL-4)of children were compared.Results:Before treatment,the level of immunoglobulin G(IgG)in the pneumonia group was lower than that in the upper respiratory tract infection group and bronchitis group(P<0.05).3 months after treatment,the levels of IgG,CD4+,CD4+/CD8+in the three groups were higher than those before treatment,and the level of CD8+was lower than that before treatment(P<0.05).Before treatment,the level of IL-4 in the pneumonia group was higher than that in the upper respiratory tract infection group and bronchitis group(P<0.05).3 months after treatment,the levels of IL-4 in the three groups were lower than those before treatment(P<0.05).Conclusion:Spleen aminopeptide can improve immunologic indexes and body resistance of children in the treatment of pediatric RRTI,and the effect is more obvious in children with recurrent pneumonia,which is worthy of clinical application.
