仙方活命饮内服外敷与造口术治疗前庭大腺脓肿疗效比较

摘　　要：目的:探讨仙方活命饮外敷与前庭大腺脓肿造口术治疗前庭大腺脓肿的效果及对复发率的影响。方法:选取瑞昌市中医医院2017年1月至2019年12月收治的80例前庭大腺脓肿患者,根据治疗方法不同分为对照组38例,观察组42例,对照组使用前庭大腺脓肿造口术治疗,观察组使用仙方活命饮内服和外敷治疗。记录总有效率、复发率,治疗前后使用视觉模拟评分法(VAS)评定患者疼痛程度,检测炎性反应指标,使用简明健康量表(SF-36)评估生活质量。结果:两组治疗总有效率均达到90%以上,差异无统计学意义(P>0.05);观察组平均治愈时间、性交疼痛率、1年复发率均低于对照组(P<0.05);治疗前两组VAS评分比较,差异无统计学意义(P>0.05),治疗后观察组VAS评分低于对照组(P<0.05);治疗前两组血清白细胞介素-1(IL-1β)、肿瘤坏死因子-α(TNF-α)比较,差异无统计学意义(P>0.05),治疗后观察组IL-1β、TNF-α均低于对照组(P<0.05)。治疗前两组SF-36量表中社会功能、情感职能、精神健康、生理职能比较,差异无统计学意义(P>0.05),治疗后观察组各维度评分均高于对照组(P<0.05)。结论:仙方活命饮内服和外敷与前庭大腺脓肿造口术均能有效治疗前庭大腺脓肿,但前者治愈速度快,且能有效降低复发率、疼痛程度和炎症反应,提高生活质量。Objective:To explore the effect of external application of Xianfang Huoming Yin and colostomy for the treatment of vestibular gland abscess and its influence on the recurrence rate.Methods:80 cases of patients with vestibular gland abscess in Ruichang Hospital of Traditional Chinese Medicine from January 2017 to December 2019 were selected and divided into control group(38 cases)and observation group(42 cases)according to different treatment methods.The control group was treated with vestibular gland abscess ostomy,and the observation group was treated with Xianfang Huoming Yin.The total effective rate and recurrence rate were recorded.Visual analogue scale(VAS)was used to evaluate the degree of pain,inflammatory reaction indexes were detected before and after treatment,and the quality of life was evaluated by short form health scale(SF-36).Results:The total effective rate of the two groups were more than 90%,the difference was not statistically significant(P>0.05);the average cure time,sexual intercourse pain rate and 1-year recurrence rate of the observation group were lower than those of the control group(P<0.05);there was no significant difference in VAS score between the two groups before treatment(P>0.05),but the VAS score of the observation group was lower than that of the control group after treatment(P<0.05);before treatment,there was no significant difference in serum interleukin-1(IL-1β)and tumor necrosis factor-α(TNF-α)between the two groups,after treatment,IL-1βand TNF-αin the observation group were lower than those in the control group(P<0.05).Before treatment,there was no significant difference in social function,emotional function,mental health and physiological function between the two groups(P>0.05).After treatment,the scores of each dimension in the observation group were higher than those in the control group(P<0.05).Conclusion:Both Xianfang Huoming Yin applied internally and externally and neostomy of Bartholin's gland abscess are effective in the treatment of abscess of vestibular gland
