摘 要:目的了解传染病医院住院患者标本分离肠杆菌分布及耐药趋势,为临床合理应用抗菌药物提供依据.方法采用细菌分离鉴定和药敏试验方法,对河南省部分传染病医院住院患者送检临床标本进行病原菌分离鉴定和药敏试验.结果从4家传染病医院共收集临床标本分离的肠杆菌科细菌3467株,以肺炎克雷伯菌、大肠埃希菌和奇异变形杆菌为主.肠杆菌科主要分离自痰液、血液、灌洗液等标本,标本来源以ICU、艾滋病病区、神经外科等为主.传染病医院临床分离的肠杆菌对青霉素类、头孢菌素类、喹诺酮类及磺胺类药物耐药率较高,对碳青霉烯类、替加环素及含酶抑制剂的抗菌药物较敏感.多重耐药菌以产ESBLs菌株、CRE菌株较多.结论调查的4家传染病医院临床标本中肠杆菌科分离率较高,对常用抗菌药物普遍耐药,应加强耐药菌监测.Objective To understand the distribution and drug resistance trend of Enterobacteriaceae isolated from inpatients in infectious disease hospitals,so as to provide a basis for the reasonable clinical application of antimicrobials.Methods Using the methods of bacterial isolation and identification and drug susceptibility testing,clinical specimens of inpatients in some infectious disease hospitals in Henan Province were tested for pathogen isolation and identification and drug susceptibility.Results A total of 3467 strains of Enterobacteriaceae isolated from clinical specimens were collected from 4 infectious disease hospitals,mainly Klebsiella pneumoniae,Escherichia coli and Proteus mirabilis.Enterobacteriaceae were mainly isolated from sputum,blood,lavage fluid and other specimens.The sources of specimens were mainly from ICU,AIDS ward and neurosurgery department.Enterobacteriaceae isolated clinically from infectious disease hospitals were resistant to penicillins,cephalosporins,quinolones and sulfonamides,and were more sensitive to carbapenems,tigecycline and antibacterial drugs containing enzyme inhibitors.The multidrug-resistant organisms were mostly ESBLsproducing strains and CRE strains.Conclusion The clinical specimens of the 4 infectious disease hospitals investigated have a high isolation rate of Enterobacteriaceae,and they are generally resistant to commonly used antibacterial drugs.The surveillance of drugresistance bacteria should be strengthened.
