自制下鼻甲骨骨折外移器应用于慢性肥厚性鼻炎中的价值分析

摘　　要：目的分析自制下鼻甲骨骨折外移器对慢性肥厚性鼻炎的治疗效果,为临床应用提供依据。方法选择2018年6月—2019年6月于九江市中医医院接受治疗的慢性肥厚性鼻炎患者共80例,采用自制的下鼻甲骨骨折外移器进行手术治疗。记录手术时间、术中出血量、术后鼻腔疼痛程度的视觉模拟评分法(visual analogue scale,VAS)评分、术后并发症、治疗总有效率与术后复发状况,对比治疗前后的黏液纤毛传输速率。结果 80例患者的平均手术时间为(32.31±5.46)min,平均术中出血量为(6.28±1.06)m L;不同时点的平均VAS评分相比,差异有统计学意义(P<0.05);1例患者发生鼻腔粘连,1例患者发生嗅觉减退,并发症发生率=2.50%;术后80例患者的治疗总有效率=97.50%。术后80例患者的平均黏液纤毛传输速率比术前短,差异有统计学意义(P<0.05)。术后为期6个月的随访无复发情况。结论自制下鼻甲骨骨折外移器治疗慢性肥厚性鼻炎的临床疗效确切,能有效改善患者的黏液纤毛传输功能,且具有手术时间短、术中出血少、术后疼痛轻、术后并发症发生率与复发率低的优点。Objective To analyze the therapeutic effect of a self-made inferior turbinate fracture external transfer device on chronic hypertrophic rhinitis,and provide a basis for clinical application.Methods A total of 80 patients with chronic hypertrophic rhinitis who were treated in our hospital from June 2018 to June 2019 were selected for surgical treatment with a self-made inferior turbinate fracture extension device.Results the operation time,intraoperative blood loss,postoperative nasal pain degree of visual analogue scale(VAS)score,postoperative complications,total effective rate of treatment and postoperative recurrence,and compare the mucociliary transmission before and after treatment rate.Results The average operation time of 80 patients was(32.31±5.46)min,and the average intraoperative blood loss was(6.28±1.06)ml.Compared with the average VAS scores at different time points,the difference was statistically significant(P<0.05);Nasal adhesions occurred in 1 patient,hypoosmia occurred in 1 patient,and the complication rate was 2.50%;the total effective rate of treatment in 80 patients after surgery was 97.50%.The average mucociliary transmission rate of 80 patients after operation was shorter than that before operation,and the difference was statistically significant(P<0.05).A 6-month follow-up investigation found that 80 patients had no recurrence.Conclusion The self-made inferior turbinate fracture external transfer device has a definite clinical effect in the treatment of chronic hypertrophic rhinitis.It can effectively improve the patient’s mucociliary transmission function,and has short operation time,less intraoperative bleeding,light postoperative pain,and postoperative complications The advantages of low incidence and recurrence rate.
