摘 要:目的分析黑龙江省HIV-1感染者的毒株亚型构成、分子簇形成和耐药株流行情况。方法对2018年1-6月期间新诊断的、尚未进行抗病毒治疗的黑龙江省HIV-1感染者,采集血样、提取病毒RNA,采用反转录聚合酶链式反应(RT-PCR)/巢式PCR方法扩增HIV-1 pol基因,测序后进行生物信息学分析。结果获得157例HIV-1样本的pol基因序列,检测出7种毒株亚型,以CRF01_AE占比最高,为61.8%(97例),其次为CRF07_BC,占20.4%(32例);21条基因序列形成了10个可能具有传播关系的分子簇,成簇率13.4%;总体耐药率10.2%(16/157),其中NNRTI、NRTI、PI的耐药率分别为5.1%(8/157)、1.3%(2/157)和3.8%(6/157)。结论黑龙江省HIV-1的毒株亚型以CRF01_AE亚型4簇和5簇为主,独特重组毒株URF_01B及A1亚型成簇传播。对治疗前耐药问题应给予密切关注。Objective To understand the subtype composition,molecular cluster formation,and prevalence of drugresistant strains of human immunodeficiency virus-1(HIV-1)infections in Heilongjiang province.Methods A crosssectional survey was conducted among HIV/AIDS patients who were newly diagnosed and without therapy in Heilongjiang province from January to June in 2018.Blood samples were collected to analyze HIV subtypes,molecular transmission cluster,and drug resistance.Results A total of 157 pol gene sequences were obtained,and seven subtypes of strains were detected,with CRF01_AE predominant position accounting for 61.8%(97/157),followed by CRF07_BC 20.4%(32/155).Ten molecular clusters were formed among 21 sequences,with a clustering rate of 13.4%(21/157).The drug resistance rate for pretreatment was 10.2%(16/157),including 5.1%(8/157)for NNRTI,1.3%(2/157)for NRTI,and 3.8%(6/157)for PI.Conclusion The subtypes of HIV-1 were mainly cluster 4 and cluster 5 in CRF01_AE,with URF_01B and subtyped A1 transmitting in the cluster in Heilongjiang province.The problem of drug resistance of Pretrea tment should be concerned.
