丁樱教授基于“肾络癥积”治疗儿童紫癜性肾炎伴新月体病变的中医思路

摘　　要：丁樱教授认为,过敏性紫癜急性发作后期,若免疫复合物或各种炎性因子未能及时有效地清除,反复迁延,则会累积于肾,沉积于毛细血管网,发展为紫癜性肾炎,即为"邪伏肾络"。并认为紫癜性肾炎的前期主要为伏火扰动,损伤肾络;后期火热之邪内舍于血,血热煎灼津液成瘀;肾络受损,代谢水液功能失常,聚湿成痰;热邪不解,与湿相搏,则湿热胶着,缠绵黏滞;痰热湿瘀日久凝滞于肾络,形成癥积,即出现西医所谓新月体形成。治疗当以固培消癥为主,清热活血通络、逐痰消癥散结、培元固本为法。Professor DING Ying holds that at the later stage of acute attack of Henoch Schonlein purpura(HSP),if immune complexes or various inflammatory factors can not be effectively and timely eliminated,with repeatedly protracted course,then it will accumulate in the kidney,deposit in the capillary network,and develop into purpura nephritis,namely"latent pathogenic factors in kidney collaterals".She also holds that in the early stage of Henoch Schonlein purpura nephritis(HSPN),it is mainly disturbance of latent fire and impairment of kidney collaterals;At the later stage,the pathogenic factors of fire and heat interiorly transmit and invade into blood,then blood-heat scorches body fluid into stasis;Impairment of kidney collaterals causes dysfunction of water fluid metabolization and dampness accumulation into phlegm;Unrelief pathogenic heat mutually contends with dampness,which results in a stalemate between dampness and heat,viscous and lingering;Prolonged phlegm-heat and damp stagnation coagulate in the kidney collaterals,forming symptom of accumulation,namely crescent formation.The treatment should be based on consolidating and reinforcing to remove accumulation by taking the following methods:clearing away heat,activating blood circulation and dredging collaterals,dispelling phlegm,removing accumulation and eliminating stagnation as well as banking up primordial qi and consolidating vital base.
