摘 要:目的:对比分析尿素-精氨酸肉汤显色法、实时荧光定量PCR方法(FQ-PCR)及RNA恒温扩增实时荧光方法(SAT)检测解脲脲原体(Uu)结果的一致性和最低检测下限。方法:采集300例疑似Uu感染患者的生殖道分泌物标本。标本同时用尿素-精氨酸肉汤显色法、FQ-PCR方法及SAT方法检测Uu。采用Kappa检验分析三种方法检测结果的一致性。阳性样本经1:20连续稀释,三种方法检测并分析最低检测下限。评估9种细菌污染对尿素-精氨酸肉汤显色法检测Uu结果干扰。结果:尿素-精氨酸肉汤显色法、FQ-PCR方法和SAT方法 Uu阳性检出率分别为19.33%、30.67%和33.00%。SAT方法与尿素-精氨酸肉汤显色法、SAT方法与FQ-PCR方法和FQ-PCR法与尿素-精氨酸肉汤显色法检测结果的总一致率分别为76.33%、85.67%和82.00%,检测结果的一致性分别为较弱(kappa值为0.40)、高度一致(kappa值为0.67)和中度一致(kappa值为0.53)。三种方法中SAT方法检测下限最低,敏感性最高。大肠埃希菌和肺炎克雷伯氏菌污染可引起尿素-精氨酸肉汤培养判断Uu的假阳性。结论:三种方法中SAT方法检测Uu的敏感性最高,其与FQ-PCR方法检测结果高度一致,阳性结果提示现症感染,临床应用价值更高。[Objective]To analyze 3 laboratory methods for detection of Uu by analyzing their consistencyandminimum detection threshold.[Method]300 discharge samples were collected and detectedby using urea-arginine broth culture Fluorescent quantitation PCR( FQ-PCR) and Simultaneous amplification and testing( SAT). Consistency of detection results and minimum detection threshold of three methods described above were analyzed by Kappa test and 1: 20 multiple dilution. The effect of 9 bacterial contamination on Uu result using urea-arginine broth culturewasevaluated. [Result]With urea-arginine broth color method,FQ-PCR method and SAT method,thepositive rates of Uu were19. 33%,30. 67% and 33. 00%,respectively. The consistence rate among SAT and urea-arginine broth culture,SAT and FQ-PCR,FQ-PCR and urea-arginine broth culturewere76. 33%,85. 67% and 82. 00%,respectively,which showed weak consistency( Kappa = 0. 40),highly consistency( Kappa =0. 67) andmoderately consistency( Kappa =0. 53).Among three methods,SAT showed the highest sensitivity due to minimum detection threshold. The contamination of E. coli and K. pneumonia could cause the false positive of Uu detected by using urea-arginine broth culture.[Conclusion]Among three methods,SAT showed the highestsensitivity and highly consistencewith FQ-PCR. Positive results suggested a symptomatic infection. SAT has higher clinical value than urea-arginine broth culture and FQ-PCR.
