血栓通注射液对下肢深静脉血栓患者凝血功能及并发症的影响

摘　　要：目的:观察血栓通注射液治疗下肢深静脉血栓(DVT)患者的疗效。方法:选取150例DVT患者,将其分为观察组与对照组各75例。对照组给予低分子肝素治疗,观察组在此基础上加血栓通注射液治疗。观察2组临床疗效,比较2组溃疡面积、视觉模拟评分(VAS)、Wells量化评分、活化部分凝血活酶时间(APTT)、D-二聚体(D-D)、血清白细胞介素-8(IL-8)、单核细胞黏附因子-1(ICAM-1)、超敏C-反应蛋白(hs-CRP)变化及并发症发生情况。结果:治疗后,观察组总有效率为93.33%,高于对照组的81.33%(P<0.05)。观察组深静脉瓣膜功能不全及下肢浅静脉曲张发生率均低于对照组(P<0.05)。与同组治疗前比较,治疗后2组溃疡面积减小(P<0.05),VAS评分、Wells量化评分及D-D、IL-8、ICAM-1、hs-CRP水平均降低(P<0.05),APTT增加(P<0.05);与对照组治疗后比较,观察组治疗后溃疡面积较小(P<0.05),VAS评分、Wells量化评分及D-D、IL-8、ICAM-1、hs-CRP水平均较低(P<0.05),APTT较高(P<0.05)。结论:血栓通注射液治疗DVT疗效显著,能有效缓解患者临床症状及体征,改善凝血功能及炎症反应,降低深静脉瓣膜功能不全及下肢浅静脉曲张的发生率。Objective:To observe the effect of Xueshuantong injection on deep vein thrombosis(DVT)of lower limbs.Methods:A total of 150 DVT patients were selected and divided into the observation group and the control group,75 cases in each group.The control group was treated with low molecular weight heparin and the observation group was additionally treated with Xueshuantong injection based on the treatment of the control group.The clinical effect in the two groups was observed.The changes of ulcer area,Visual Analogue Scale(VAS),Wells Quantitative Score,activated partial thrombin time(APTT),D-dimer(D-D),serum interleukin-8(IL-8),intercellular adhesion molecule-1(ICAM-1)and hypersensitive Creactive protein(hs-CRP)and the incidence of complications were compared between the two groups.Results:After treatment,the total effective rate was 93.33%in the observation group,higher than that of 81.33%in the control group(P<0.05).The incidence of deep vein valve insufficiency and lower extremity varicose veins in the observation group was lower than that in the control group(P<0.05).Compared with the same group before treatment,the ulcer area was decreased in the two groups after treatment(P<0.05);the VAS score,Wells Quantitative Score and levels of D-D,IL-8,ICAM-1 and hs-CRP were all decreased(P<0.05),and APTT levels were increased(P<0.05).Compared with the control group after treatment,the ulcer area was smaller in the observation group(P<0.05);the VAS score,Wells Quantitative Score and levels of D-D,IL-8,ICAM-1 and hs-CRP were all lower(P<0.05),and APTT levels were higher(P<0.05).Conclusion:Xueshuantong injection has significant curative effect on DVT patients,which can effectively relieve the clinical symptoms and signs,improve the coagulation function and inflammatory response,and reduce the incidence of deep vein valve insufficiency and superficial varicose veins of lower limbs.
