摘 要:目的探讨高温高湿环境下高强度军事训练对肾脏的损伤作用。方法选400名南沙守礁官兵进行10 km武装越野,均在训练前后及休息24 h后留尿标本进行尿液常规检查,以酶联免疫吸附法(ELISA)检测尿N-乙酰-β-D-氨基葡萄糖苷水解酶(NAG)和尿视黄醇结合蛋白(RBP);留静脉血查肌苷及尿素氮。对各项检验结果进行不同军龄及不同训练环境的对比分析。结果高温高湿组训练后尿量与常温常湿组比较有显著差异,训练后全体官兵尿潜血阳性发生率为5.75%、蛋白尿发生率为38.5%、尿NAG异常率为59.2%、尿RBP异常率为61.2%。但4组训练前后血清肌酐、尿素氮稍升高,但差异无统计学意义(P>0.05)。休息24 h后,4组除高温高湿新兵组和高温高湿老兵组尿蛋白阳性例数各1例,其余均正常。休息24 h后,4组尿NAG、尿PBP均恢复正常范围。结论高温高湿环境下训练对官兵肾脏有影响,新兵改变更明显,已热习服的老兵表现较好的热耐受力和适应性,训练后应注意休息,可以避免肾脏二次损伤。Objective To investigate the impact of high intensity military training on kidney injury in hot and humid environment.Methods A total of 400 commissioned and enlisted who stationed on Nansha Islet were enrolled for a 10-km across-country run in fully armed gear.Urine samples were collected both before training and 24 hours after training to have routine urinalysis.N-acetyl-β-D-glucosaminidase(NAG)and retinol binding protein(RBP)were detected by enzyme linked immunosorbent assay(ELISA),and venous blood samples were obtained to detect contents of creatinine(Cr)and blood urea nitrogen(BUN).The detection results were compared and analyzed against different training environment for those people with different service years.Results Upon completion of training,significant differences could be noted in the output of urine,when comparisons were made between the high temperature and humidity group and the routine temperature and humidity group.Following training,the rates of hematuria,proteinuria,NAG,and RBP were respectively 5.75%,38.5%,59.2%and 61.2%.The contents of Cr and BUN detected before and after training in the 4 groups slightly increased,but without statistical significance(P>0.05).Twenty-four hours after rest,the level of urinary protein in the 4 groups returned to normal,except that there was only one positive case in the high temperature and humidity recruit group and the high temperature and humidity veteran group.After 24 hours of rest,the levels of NAG and PBP in the 4 groups all returned to normal.Conclusion Training under high temperature and humidity environment had certain impact on the renal function of trainees,the recruits in particular.The acclimatized veterans displayed better heat tolerance and adaptation.After training,trainees should have good rest,so as to avoid secondary injury to kidney.
