贝那普利联合左旋卡尼汀治疗心力衰竭患者的临床效果

摘　　要：目的探讨贝那普利联合左旋卡尼汀治疗心力衰竭患者的临床效果。方法选取2019年8月—2020年8月广州市白云区第二人民医院收治的68例心力衰竭患者作为研究对象,按照随机数字表法将其分为对照组与研究组,每组各34例。对照组患者应用贝那普利治疗,研究组患者应用左旋卡尼汀+贝那普利治疗。比较两组患者治疗前后的心功能指标[左室舒张末期容量(LVEDV)、左心室射血分数(LVEF)以及心输出量(CO)]、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、血浆N端脑钠肽前体(NT-proBNP)水平以及治疗后的6 min内步行距离。结果治疗后,研究组患者的LVEDV水平低于对照组,LVEF、CO水平高于对照组,差异有统计学意义(P<0.05)。研究组患者治疗8、16 d后的6 min内步行距离长于对照组,差异有统计学意义(P<0.05)。治疗后,研究组患者的NT-proBNP、IL-6、TNF-α水平均低于对照组,差异有统计学意义(P<0.05)。结论心力衰竭应用左旋卡尼汀+贝那普利的效果较好,可以改善患者的心功能,提高运动耐力,降低机体中的炎症反应,提高治疗效果。Objective To investigate the clinical effect of Benazepril combined with L-carnitine in the treatment of patients with heart failure.Methods A total of 68 patients with heart failure who were addmited in the Second People′s Hospital of Baiyun District from August 2019 to August 2020 were selected as subjects.They were divided into the control group and the study group by random number table,with 34 cases in each group.The control group was treated with Benazepril,and the study group was treated with L-carnitine+Benazepril.Cardiac function indexes(left ventricular end-diastolic volume[LVEDV],left ventricular ejection fraction[LVEF]and cardiac output[CO]),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),plasma N-terminal brain natriuretic peptide precursor(NT-proBNP)levels and walking distance within 6 minutes before and after treatment were compared between the two groups.Results After treatment,the LVEDV level of the study group was lower than that of the control group,and the levels of LVEF and CO were higher than those of the control group,and the differences were statistically significant(P<0.05).In the study group,the walking distance within 6 min after 8 and 16 days of treatment was longer than that in the control group,and the difference was statistically significant(P<0.05).The levels of NT-proBNP,IL-6,and TNF-αin the study group were lower than those in the control group after treatment,and the differences were statistically significant(P<0.05).Conclusion The application of L-carnitine+Benazepril for heart failure has a better effect,which can improve the patient′s heart function,increase exercise endurance,reduce the inflammatory response in the body,and improve the treatment effect.
