老年高血压患者血浆Klotho蛋白、sST2和Gal-3水平表达及与左心室肥厚的相关性分析

摘　　要：目的探讨老年高血压患者血浆Klotho蛋白、可溶性致癌抑制因子2(sST2)和半乳糖凝集素-3(Gal-3)水平表达及与左心室肥厚(LVH)的相关性。方法选取2019年1月至2020年1月于安康市中医医院收治的96例老年高血压患者,根据是否合并LVH分为非左心室肥厚(NLVH)组(52例)和LVH组(44例),另选取同期我院体检健康者50例为对照组,测定所有对象血浆Klotho蛋白、sST2和Gal-3水平,行超声心动图(UCG)检查,计算左室质量指数(LVMI),对比三组研究对象血浆Klotho蛋白、sST2和Gal-3水平和LVMI,Pearson相关性分析血浆Klotho蛋白、sST2和Gal-3水平与LVMI的相关性,多因素Logistics回归分析LVH危险因素。结果LVH组血浆Klotho蛋白水平明显低于NLVH组和对照组,血浆sST2、Gal-3水平和LVMI明显高于NLVH组和对照组,NLVH组Klotho蛋白水平明显低于对照组,血浆sST2、Gal-3水平和LVMI明显高于对照组(P<0.05);Pearson相关分析显示,血浆Klotho蛋白水平与LVMI呈负相关,sST2和Gal-3水平与LVMI呈正相关(P<0.05);多因素Logistics回归分析显示,低Klotho蛋白和高sST2、Gal-3水平为LVH独立危险因素(P<0.05)。结论老年高血压患者血浆Klotho蛋白明显降低,sST2和Gal-3水平上升,与LVH有良好相关性,早期监测有利于LVH预防、诊断、预后。Objective To discuss the correlation between expressions of plasma Klotho protein,soluble suppression of tumorigenicity 2(sST2)and galectin-3(Gal-3)and left ventricular hypertrophy(LVH)in elderly patients with high blood pressure(HBP).Methods The elderly HBP patients(n=96)were chosen from Hospital of Traditional Chinese Medicine of Ankang City from Jan.2019 to Jan.2020,and then divided,according to whether they had complicated LVH or not,into NLVH group(n-52)and LVH group(n=44).The healthy controls with physical examinations were chosen into control group(n=50).The levels of plasma Klotho protein,sST2 and Gal-3 were detected in all groups.All groups were given echocardiogram examination for calculating left ventricular mass index(LVMI).The levels of plasma Klotho protein,sST2 and Gal-3 and LVMI were compared among 3 groups.The correlation between levels of plasma Klotho protein,sST2 and Gal-3 and LVMI was analyzed by using Pearson correlation analysis.The risk factors of LVH were analyzed by using multi-factor Logistics regression analysis.Results The level of plasma Klotho protein was significantly lower and levels of plasma sST2 and Gal-3 and LVMI were significantly higher in LVH group than those in NLVH group and control group.The level of plasma Klotho protein was significantly lower and levels of plasma sST2 and Gal-3 and LVMI were significantly higher in NLVH group than those in control group(P<0.05).The results of Pearson correlation analysis showed that Klotho protein level was negatively correlated to LVMI,and sST2 and Gal-3 levels were positively correlated to LVMI(P<0.05).The results of multi-factor Logistics regression analysis showed that lower Klotho protein level and higher sST2 and Gal-3 levels were independent risk factors of LVH(P<0.05).Conclusion The level of plasma Klotho protein decreases significantly and levels of sST2 and Gal-3 increase,which are well correlated to LVH in elderly HBP patients.The early monitoring of LVH is beneficial to the prevention,diagnosis and prognosis of LVH.
