他汀类药物干预后心房颤动患者血清炎性标志物水平变化分析

摘　　要：目的探讨他汀类药物在高血压合并孤立性心房颤动(IAF)患者中的应用价值。方法选取2018年5月至2020年5月于河北北方学院附属第一医院心血管内科240例高血压合并首诊IAF患者进行临床观察,按随机数字表法分为对照组和观察组,每组各120例。对照组采用常规药物,包括降压药、控制心率、抗凝药等;观察组在此基础上加用他汀类药物(瑞舒伐他汀钙片20 mg,1/晚)。比较两组患者治疗前及治疗后3个月血脂相关指标(三酰甘油,低密度脂蛋白胆固醇,总胆固醇)及超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)的变化,以及治疗后6个月窦性心律维持情况。结果治疗后3个月观察组的血脂相关指标及hs-CRP、TNF-α水平均较治疗前显著改善(P<0.05),与对照组比较,差异具有统计学意义(P<0.05);对照组治疗前后则无显著差异(P>0.05)。两组治疗后6个月观察组窦性心律维持率87.9%(105/120)vs.对照组68.5%(82/120),差异有统计学意义(P=0.000)。结论他汀类药物降脂同时具有抗炎作用,可降低心房颤动(房颤)患者的炎性标志物水平,提升窦性心律维持率,有助于降低房颤的复发。Objective To discuss the application value of statins in patients with hypertension complicated by isolated atrial fibrillation(IAF).Methods The patients with hypertension complicated by IAF(n=240)were chosen from Department of Cardiology in the First Affiliated Hospital of Hebei North University from May 2018 to May 2020,and divided,according to random digital table,into control group and observation group(each n=120).The control group was treated with routine drugs including antihypertensive drugs,heart rate control drugs and anticoagulant drugs.The observation group was additionally given statins[rosuvastatin calcium tablets(20 mg),once a night].The changes of indexes related to blood fat including triglyceride(TG),low-density lipoprotein-cholesterol(LDL-C),total cholesterol(TC),high sensitivity C-reactive protein(hs-CRP)and tumor necrosis factor-α(TNF-α)before treatment and 3 months after treatment,and maintenance rate of sinus rhythm 6 months after treatment were compared in 2 groups.Results The indexes related to blood fat and levels of hs-CRP and TNF-αwere significantly improved in observation group after treatment for 3 months(P<0.05),and difference had statistical significance between 2 groups(P<0.05).The control group had no significant changes before and after treatment(P>0.05).The maintenance rate of sinus rhythm was 87.9%(105/120)in observation group and 68.5%(82/120)in control group after treatment for 6 months(P=0.000).Conclusion Statins have both anti-inflammatory and lipid-lowering effects,can reduce the levels of inflammatory markers,improve the maintenance rate of sinus rhythm,and decrease IAF recurrence.
