摘　　要：探讨股骨近端防旋髓内钉(PFNA)对老年股骨转子间骨折患者髋关节功能恢复的影响。方法:选择我院收治的90例老年股骨转子间骨折患者,按随机对照原则分为两组,各45例。对照组给予传统动力髋螺钉(DHS)治疗,观察组给予PFNA治疗。比较两组患者手术情况、术前及术后6个月髋关节功能评分(Harris评分)情况。结果:观察组术中出血量少于对照组,手术、骨折愈合、住院、术后疼痛缓解时间均短于对照组,差异有统计学意义(P<0.05);术后6个月,两组疼痛、功能、畸形、运动范围、总分均高于治疗前,观察组高于对照组,差异有统计学意义(P<0.05)。结论:PFNA能够有效缩短老年股骨转子间骨折患者手术时间,减少术中出血,缩短患者康复时间,改善髋关节功能。Objective To investigate the effect of intertrochanteric nail on the recovery of femoral intertrochanteric joint function in elderly patients.Method 90 elderly patients with intertrochanteric fracture in our hospital were randomly divided into two groups,45 cases in each group.The control group was treated with traditional dynamic hip screw(DHS),and the observation group was treated with proximal femoral nail antirotation(PFNA).The operation condition,Harris score before and 6 months after operation were compared between the two groups.Results The intraoperative blood loss in the observation group was less than that in the control group,the operation,fracture healing,hospitalization and postoperative pain relief time were shorter than those in the control group(P<0.05);6 months after operation,the pain,function,deformity,range of motion,total score of the two groups were higher than before treatment,and the observation group was higher than the control group(P<0.05).Conclusion Proximal femoral nail antirotation can effectively shorten the operation time of elderly patients with intertrochanteric fracture,reduce intraoperative bleeding,shorten the recovery time of patients,and improve hip joint function.
