改良气管内吸痰护理在机械通气新生儿呼吸道管理中的研究

摘　　要：目的:探讨改良气管导管内吸痰护理在机械通气新生儿中降低呼吸机相关性肺炎(VAP)等不良反应,减少机械通气治疗时间的临床效果。方法:将58例新生儿呼吸窘迫综合征进行呼吸机治疗患儿随机分为试验组和对照组,各29例。对照组按常规实施按时吸痰,试验组按改良气管插管内吸痰护理方法按需吸痰。结果:试验组VAP发生、患儿痰阻、经皮动脉血氧饱和度(SpO_(2))下降至80%以下(以下简称SpO_(2)下降)等不良反应及机械通气时间少于对照组,差异均有统计学意义(P<0.05)。结论:应用改良气管插管内吸痰护理方法,可降低VAP发生、患儿痰阻、SpO_(2)下降等不良反应及减少机械通气治疗时间。Objective:To explore the clinical effect of modified endotracheal tube sputum suction nursing in reducing the adverse reactions such as the occurrence of ventilator-associated pneumonia(VAP)and the treatment time of mechanical ventilation in mechanically ventilated newborns.Methods:A total of 58 cases of neonatal respiratory distress syndrome were randomly divided into experimental group(n=29)and control group(n=29).The control group received regular sputum suction in time,while the experimental group was given sputum suction as needed according to the improved nursing method of endotracheal intubation.Results:Adverse reactions such as the occurrence of VAP,sputum obstruction,blood oxygen saturation(SPO_(2))decreased to below 80%(hereinafter referred to as SpO_(2) decrease)and the time of mechanical ventilation in the experimental group were less than those in the control group,the differences were statistically significant(P<0.05).Conclusion:The improved nursing method of sputum suction through endotracheal intubation can reduce the adverse reactions such as the occurrence of VAP,sputum obstruction,blood oxygen saturation(SpO_(2)),and reduce the treatment time of mechanical ventilation,which is worth popularizing in clinical nursing.
