摘　　要：目的:探究促甲状腺激素(TSH)及抗甲状腺过氧化物酶抗体(TPOAb)检测在育龄妇女优生优育中的价值。方法:选取在东莞市松山湖中心医院孕检的育龄期妇女1887名作为研究对象,检测TSH、TPOAb水平。结果:1887名育龄期妇女中,TSH水平异常率为143/1887(7.6%),其中40~45岁TSH异常率高于20~29岁及30岁~39岁(P<0.05);BMI偏低的育龄期妇女TSH异常率低于BMI偏高育龄期妇女(P<0.05);TSH异常的育龄期妇女中TPOAb阳性检出率高于TSH正常的育龄期妇女(P<0.05)。结论:40~45岁、BMI偏高或偏低的育龄期妇女,孕前应注重TSH、TPOAb筛查,及早发现诊断甲状腺功能异常,以降低妊娠风险。Objective:To investigate the value of thyroid-stimulating hormone(TSH)and anti-thyroid peroxisome antibody(TPOAb)in prenatal and postnatal care for women of childbearing age.Methods:A total of 1887 women of childbearing age who had undergone pregnancy check-up at Songshan Lake Central Hospital in Dongguan were included for measurement of TSH and TPOAb levels.Results:Among the 1887 women of childbearing age,abnormal TSH level was found in 143 out of 1887(7.6%).Women aged between 40 and 45 years were more likely to have abnormal TSH level compared with those aged between 20 and 29 years or those aged between 30 and 39 years(P<0.05).Low-BMI women of childbearing age were less likely to have abnormal TSH level compared to those with high BMI(P<0.05).The positive rate of TPOAb was higher in women of childbearing age with abnormal TSH level compared to those with normal TSH level(P<0.05).Conclusion:Women of childbearing age between 40 to 45 years old and those with a high or low BMI should pay attention to TSH and TPOAb screening before pregnancy,so as to identify and diagnose thyroid function abnormality early and reduce high-risk pregnancy.
