肱骨远端后外侧钢板联合微型钢板治疗肱骨下段骨折的经验总结

摘　　要：总结探讨肱骨远端后外侧锁定钢板联合微型钢板治疗肱骨下段骨折的经验。肱骨下段骨折,尤其是粉碎性骨折,临床治疗困难,极易遗留肘关节功能障碍、异位骨化、骨折不愈合等并发症。手术治疗是主要的治疗方法,钢板固定是金标准,但单钢板固定生物力学稳定性差,骨折可能再移位。而采用肱骨远端后外侧钢板联合微型钢板治疗肱骨下段骨折平均骨性愈合时间为7~15个月,无骨折不愈合、内固定物失效、肘关节强直僵硬等并发症。通过总结我们认为采用肱骨远端后外侧钢板联合微型钢板治疗肱骨下段骨折的疗效确切,愈合率高,固定牢靠,可尽早的功能锻炼,临床效果满意。To summarize and explore the experience of the treatment of lower humeral fractures with locking plate and microplate on the posterolateral distal humerus.Fractures of the lower part of the humerus,especially comminuted fractures,are difficult to clinically treat,and it is easy to leave elbow joint dysfunction,heterotopic ossification,fracture nonunion and other complications.Surgical treatment is the main treatment method,and plate fixation is the gold standard,but the biomechanical stability of single plate fixation is poor,and the fracture may be displaced again.The average osseous healing time of lower humeral fractures treated with the posterolateral plate of the distal humerus combined with the microplate was 7~15 months,and there were no complications such as nonunion of the fracture,failure of internal fixation,ankylosis and stiffness of the elbow joint.By summarizing,we believe that the treatment of lower humeral fractures with the posterolateral distal humerus plate combined with the micro-plate has a definite effect,high healing rate,firm fixation,early functional exercise,and satisfactory clinical results.
