急、慢性血糖比值对急性肾损伤长期不良预后的预测价值研究

摘　　要：目的慢性血糖比值在急性肾损伤(AKI)患者长期不良预后中的预测价值。方法入选2014年1月至2017年1月在贵州省人民医院肾内科住院的AKI患者156例,对患者进行长期随访,终点事件是死亡。所有患者住院即刻均测量静脉血糖(急性期血糖)和糖化血红蛋白(HbA_(1c))水平,用HbA_(1c)计算慢性血糖水平,按下式计算转换:慢性血糖水平=(28.7×HbA_(1c)-46.7)/18,计算急、慢性血糖比值。结果依据终点事件将患者分为好转组(n=128)及死亡组(n=28),进行多因素logistic回归分析,结果显示:2组患者比较,年龄,急、慢性血糖比为独立危险因素(OR分别为1.04、25.89,P<0.05)。结论急、慢性血糖比值为反映AKI患者长期不良预后的独立危险因素,其水平与AKI的不良预后相关。急、慢性血糖比值越高,长期预后越差。Objective To investigate the relationship between acute and chronic glycemia ratio and long-term poor prognosis in patients with AKI.Methods 156 patients with AKI in Guizhou Provincial People’s Hospital between January2014 to January 2017 were enrolled follow-up for a long period of time,the primary end point of the study was the mortality.Blood glycemia and HbA1 c levels were measured in all patients at hospital admission,average chronic glycemia levels were estimated by HbA1 c,we calculated average chronic glucose levels=[28.7×HbA1 c(%)-46.7]/18,then calculated acute and chronic glycemia ratio.Results The AKI patients were divided into the remission group(n=128)and the death group(n=28).Multivariate Logistic regression analysis showed that age、acute and chronic glycemia ratio were independent risk factors(OR=1.04、25.89,P<0.05).Conclusion Acute and chronic glycemia ratio is independent risk factors for poor long-term prognosis in patients with AKI,it levels are closely related to adverse AKI events,the higher the ratio of acute and chronic glycemia,the worse long-term prognosis.
