中国双向代际支持的变化趋势及对老年人门诊服务利用的影响研究

摘　　要：[目的]揭示中国双向代际支持近十年的变化趋势,探究双向代际支持对老年人门诊服务利用的影响。[方法]利用2010-2018年中国家庭追踪调查数据,采用描述性统计分析对双向代际支持的变化趋势进行描述;利用最新2018年数据,采用二元Logistic回归模型研究双向代际支持对老年人门诊服务利用的影响。[结果]2010-2018年,中国双向代际经济支持比例呈上升趋势,双向代际照料支持比例呈下降趋势,与子女关系亲近比例上升。有亲代经济支持(P<0.01)、有子代经济支持(P<0.01)、有亲代照料支持(P<0.05)和有子代照料支持(P<0.01)均能促进老年人就医,与子女关系亲近减少就医(P<0.01)。[结论]应当加强对传统家庭照料的重视,同时注重社会支持养老、抚幼功能的补充,促进医养结合。Objective To explore the trends of bi-directional intergenerational support in China in recent ten years and the influence of two-way intergenerational support on outpatient service utilization of the elderly.Methods It used follow up survey data of Chinese family from 2010 to 2018,descriptive statistical analysis was used to analyze the trends of two-way intergenerational support.Based on the latest data of 2018,binary Logistic regression model was used to study the impact of two-way intergenerational support on outpatient service utilization for the elderly.Results From 2010 to 2018,the proportion of two-way intergenerational economic support in China showed upward trend,while the proportion of two-way intergenerational care support showed downward trend,and the proportion of close relationship with children increased.Parental economic support(P<0.1),descendant economic support(P<0.01),parental care support(P<0.05)and descendant care support(P<0.01)can promote the elderly to see a doctor,and close relationship with children could reduced the incidence of medical treatment(P<0.01).Conclusions We should pay more attention to traditional family care and the supplement of social support for the aged and the care of the young at the same time,and promote the integrated treatment and convalesce.
