摘　　要：目的分析2013~2019年甘肃省麦积区农村汉族初产妇妊娠年龄年度变化趋势。方法选取2013~2019年麦积区参加国家免费孕前优生健康检查的农村汉族8274例初产妇为研究对象,采用简单线性回归、广义线性回归和边际效应模型分析初产妇妊娠年龄和高龄妊娠女性比例的变化趋势。结果纳入研究的初产妇妊娠年龄从2013年的(24.57±3.18)岁逐年上升至2019年的(26.74±3.57)岁,高龄妊娠率从2013年的0.68%上升至2019年的4.28%,趋势变化具有统计学意义(P<0.05)。2013~2019年间初产妇的妊娠年龄年均增加0.36岁[95%CI(0.30,0.42)],高龄妊娠率年均增长0.61%[95%CI(0.44%,0.78%)]。广义线性回归结果显示,多因素调整后,2013~2019年间初产妇妊娠年龄年均增加了0.27岁[95%CI(0.23,0.31)],高龄妊娠率年均增长31.20%[95%CI(19.03%,44.61%)],趋势具有统计学意义(P<0.05)。采用简单线性回归分析边际效应结果显示,2013~2019年间初产妇妊娠年龄边际值年均增加0.48岁[95%CI(0.32,0.65)],高龄妊娠率边际值年均增长0.27%[95%CI(0.21%,0.32%)]。结论2013~2019年甘肃省麦积区农村汉族初产妇妊娠年龄和高龄妊娠率逐年增加,提示须加强高龄产妇生殖保健服务能力建设。Objective:To analyze the prevalence trend of pregnancy age of primipara in rural women in Maiji District of Gansu Province from 2013 to 2019.Methods:A total of 8274 rural primiparous of Han nationality who participated in the National Free Pre-pregnancy Health Checkup Project in Maiji District from 2013 to 2019 were selected as the research objects.The simple linear model,generalized linear model and marginal effects model were applied to explore the trend of pregnancy age and the prevalence of elderly pregnant women.Results:Pregnancy age of primipara increased from(24.57±3.18)years in 2013 to(26.74±3.57)years in 2019(P<0.05),0.36 years[95%CI(0.30,0.42)]for annual increment.The prevalence of elderly pregnant women significantly increased from 0.68%in 2013 to 4.28%in 2019(P<0.05),0.61%[95%CI(0.44%,0.78%)]for annual growth rate.The generalized linear model and logistic regression model revealed that after adjusting for multiple factors,the average age of primiparas increased by 0.27 years[95%CI(0.23,0.31)],and the average age pregnancy rate increased by 31.20%[95%CI(19.03%,44.61%)]from 2013 to 2019,with a statistically significant trend(P<0.05).The marginal effects model also revealed that the annual increment of pregnancy age was 0.48 years[95%CI(0.32,0.65)],and the elderly pregnancy rate increased by 0.27%[95%CI(0.21%,0.32%)]from 2013 to 2019.Conclusions:From 2013 to 2019,the pregnancy age and elderly pregnancy rate of primiparas in rural areas of Maiji District of Gansu Province increased year by year,suggesting that it is necessary to strengthen the construction of reproductive health service capacity of elderly puerperas.
