中国人群中新鲜和冻融胚胎移植后复合妊娠发生率的差异:系统评价和荟萃分析

摘　　要：目的比较新鲜胚胎移植(fET)和冻融胚胎移植(FET)中复合妊娠(HP)的发生率。方法通过计算机检索PubMed、Web of Science、Cochrane library、万方数据库和中国知网数据库,检索自建库至2020年11月发表的关于胚胎移植后相关并发症的临床研究,按照建立的纳入和排除标准筛选,对纳入的研究进行质量评估和数据提取,通过RevMan 5.3和Stata 14.0软件进行Meta分析。结果最终共纳入10项原始研究,共175856个周期,HP的总体发生率为0.45%[95%CI(0.29,0.60),P<0.00001,I 2=0]。分为fET和FET两组比较时,HP的发生率无显著性差异[OR=1.22,95%CI(0.71,2.10),P=0.48;I 2=82%]。结论总体来说,HP的发生率较低,且FET和fET的HP发生率无明显差异,提示选择FET或fET并不影响HP的发生。Objective:To compare the incidence of heterotopic pregnancy(HP)between fresh embryo transfer and frozen-thawed embryo transfer(FET).Methods:The databases of PubMed,Web of Science,Cochrane library,Wanfang database and CNKI were searched by computer.According to the established exclusion and inclusion criteria,the clinical studies published about complications after embryo transfer from database establishment to November 2020 were included.Then the quality evaluation and data extraction of the included studies were performed,and meta-analysis was conducted by using RevMan 5.3 and Stata 14.0 software.Results:Ten original studies with a total of 175856 cycles were included.The HP incidence was 0.45%[95%CI(0.29,0.60),P<0.00001,I 2=0].There was no significant difference in HP rate between the fresh embryo transfer and FET[OR=1.22,95%CI(0.71,2.10),P=0.48,I 2=82%].Conclusions:In general,the incidence of HP is low.HP incidence is not significantly different between the fresh embryo transfer and FET.Choosing fresh embryo transfer or FET does not affect the occurrence of HP.
