腹腔镜下子宫腺肌病病灶切除术联合环形减张缝合术的效果

摘　　要：目的分析腹腔镜下子宫腺肌病病灶切除术联合环形减张缝合术的效果。方法选取2017年6月至2019年3月安阳市妇幼保健院收治的78例子宫腺肌病患者作为研究对象,按照随机数表法分为对照组与观察组,各39例。两组均接受腹腔镜下子宫腺肌病病灶切除术治疗,对照组接受分层单纯连续缝合法,观察组接受环形减张缝合术。比较两组手术指标(手术时间、术中失血量)、术后恢复指标(首次排气时间、首次排便时间、术后住院时间)、术前及术后6、12个月子宫体积、月经失血图(PBAC)评分、COX痛经症状量表(CMSS)评分、并发症。结果观察组手术时间、首次排气时间、首次排便时间均短于对照组,术中失血量少于对照组(P<0.05);两组术后住院时间比较,差异无统计学意义(P>0.05)。术后6、12个月,两组子宫体积、PBAC评分、CMSS评分比较,差异无统计学意义(P>0.05)。两组均未出现并发症。结论腹腔镜下子宫腺肌病病灶切除术联合环形减张缝合术能缩短患者手术时间以及术后排气、排便时间,减少术中失血量,促进子宫恢复。Objective To analyze the effect of laparoscopic adenomyosis focus resection combined with circular tension-reducing suture.Methods A total of 78 patients with adenomyosis in Anyang Maternal and Child Health Care Hospital from June 2017 to March 2019 were selected.According to the random number table method,they were divided into control group and observation group,with 39 cases in each group.The two groups underwent laparoscopic adenomyosis focus resection.Control group underwent stratified simple continuous suture,and observation group underwent circular tension-reducing suture.The operation indexes(operation time,intraoperative blood loss)and postoperative recovery indexes(first exhaust time,first defecation time,postoperative hospital stay),uterine volume,pictorial blood loss assessment chart(PBAC)score,COX menstrual symptom scale(CMSS)score before and 6,12 months after operation and complications were compared between the two groups.Results The operation time,first exhaust time and first defecation time of observation group were all shorter than those of control group,and intraoperative blood loss was less than that of control group(P<0.05).There was no statistical difference in postoperative hospital stay between the two groups(P>0.05).Six and twelve months after operation,there were no statistical differences in uterine volume,PBAC score and CMSS score between the two groups(P>0.05).Conclusion Laparoscopic adenomyosis focus resection combined with circular tension-reducing suture can shorten the operation time,postoperative exhaust and defecation time,reduce intraoperative blood loss,promote uterine recovery.
