中药方联合恩格列净对糖尿病肾病的治疗效果分析

摘　　要：目的观察中药方联合恩格列净对糖尿病肾病的治疗效果。方法选择该院2020年1—12月间64例糖尿病肾病患者,并随机分为两组:对照组32例在饮食控制、运动指导及原降糖治疗基础上给予恩格列净治疗,观察组在对照组基础上给予中药方进行治疗,连续治疗3个月后,检测两组患者的体质量指数(BMI)、空腹血糖(FPG)、餐后2 h血糖(2 hPG)、糖化血红蛋白(HbA1c)、内皮素(ET-1)、血尿酸(UA)、肌酐(Cr)、尿素氮(BUN)、24 h尿蛋白定量(24 h-UP)等指标水平,并与治疗前指标数值进行对比;观察评价两组的临床疗效及不良反应发生情况。结果治疗前,两组患者BMI、FPG、2 hPG、HbA1c、ET-1、UA、Cr、BUN、24 h-UP指标水平均差异无统计学意义(P>0.05);治疗3个月后,两组患者BMI、FPG、2 hPG、HbA1c、ET-1、UA、Cr、BUN、24 h-UP指标水平均较治疗前有所改善,且观察组改善程度优于对照组,差异有统计学意义(P<0.05);观察组临床疗效优于对照组,差异有统计学意义(P<0.05);两组患者不良反应发生率差异无统计学意义(P>0.05)。结论中药方联合恩格列净对糖尿病肾病的治疗效果显著,能明显改善患者血糖、肾功能和血管内皮功能,且具有较高的安全性。Objective To observe the therapeutic effect of traditional Chinese medicine combined with enpagliflozin on diabetic nephropathy.Methods 64 patients with diabetic nephropathy in the hospital from January to December 2020 were selected and randomly divided into two groups:32 patients in the control group were given empagliflozin on the basis of diet control,exercise guidance and original hypoglycemic therapy,the observation group was treated with traditional Chinese medicine on the basis of the control group.After 3 months of continuous treatment,the body mass index(BMI),fasting blood glucose(FPG),postprandial blood glucose(2 hPG),and glycosylated hemoglobin(HbA1c),endothelin(ET-1),blood uric acid(UA),creatinine(Cr),urea nitrogen(BUN),24 h urine protein quantitative(24 h-UP)indicators of the two groups were tested,and compared with the index values before treatment;observe and evaluate the clinical efficacy and adverse reactions of the two groups.Results Before treatment,there was no statistically significant differenced in BMI,FPG,2 hPG,HbA1c,ET-1,UA,Cr,BUN,24 h-UP,UACR between the two groups(P>0.05);after 3 months of treatment,BMI,FPG,2 hPG,HbA1c,ET-1,UA,Cr,BUN,24 h-UP of the two groups of patients improved compared with before treatment,and the improvement degree of the observation group was better than that of the control group,the difference was statistically significant(P<0.05).The clinical efficacy of the observation group was better than that of the control group,the difference was statistically significant(P<0.05);there was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The combination of traditional Chinese medicine and enpagliflozin has a significant therapeutic effect on diabetic nephropathy,which can significantly improve the blood glucose,renal function and vascular endothelial function of patients,and has high safety.
