摘 要:目的观察尿毒清联合苯扎贝特调节糖尿病肾病患者血液凝固功能及内皮功能水平的作用。方法收集青海省中医院糖尿病肾病患者120例,根据治疗不同分为对照组与观察组,每组60例。对照组患者给予苯扎贝特治疗(0.2 g/次,3次/d),观察组患者在对照组基础上给予尿毒清颗粒口服治疗(5.0 g/次,4次/d),两组患者均连续治疗4周。比较治疗前后两组患者血清中活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)、血浆凝血酶时间(TT)、纤维蛋白原(FIB)、内皮素(ET)、一氧化氮(NO)、内皮生长因子(VEGF)、超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)、丙二醛(MDA)的水平。结果治疗后两组患者血清中APTT、PT、TT、NO值水平均较治疗前升高,FIB、ET及VEGF水平均较治疗前降低,且观察组患者APTT、PT、TT、NO水平高于对照组(P<0.05),FIB、ET及VEGF水平均低于对照组(P均<0.05);治疗后观察组患者血清中SOD、GSH-Px水平均较治疗前升高,MDA水平均较治疗前降低(P均<0.05)。结论尿毒清联合苯扎贝特治疗糖尿病肾病,可改善患者血液凝固功能及内皮功能,减轻氧化应激损伤,缓解疾病进展。Objective To observe the effect of Niaoduqing combined with Benzabete on regulating blood coagulation and endothelial function in patients with diabetic nephropathy.Methods Totally 120 patients with diabetic nephropathy in our hospital were randomly divided into control group and observation group,with 60 cases in each group.The control group was given bezafibrate orally,1 tablet(0.2 g/time,3 times/d).Patients in the observation group were given Niaoduqing granules on the basis of the control group,orally,1 bag(5.0 g/time,4 times/d).Both groups were treated for 4 weeks.The levels of activated partial thromboplastin time(APTT),prothrombin time(PT),plasma thrombin time(TT),fibrinogen(FIB),endothelin(ET),nitric oxide(NO),endothelial growth factor(VEGF),superoxide dismutase(SOD),glutathione peroxidase(GSH-Px),malondialdehyde(MDA)were compared between the patients in the two groups before and after treatment.Results After treatment,the levels of APTT,PT,TT and NO in serum of the patients in the two groups were significantly higher than those before treatment,and the levels of FIB,ET and VEGF were significantly lower than those before treatment.The levels of APTT,PT,TT and NO in the observation group were higher than those in the control group(P<0.05),and the levels of FIB,ET and VEGF in the observation group were lower than those in the control group(P all<0.05).After treatment,the serum levels of SOD and GSH-Px of patients in the observation group were significantly higher than those before treatment,while the levels of MDA were significantly lower than those before treatment(P all<0.05).Conclusion Niaoduqing combined with Benzabete can significantly improve the coagulation and endothelial function,reduce oxidative stress injury and alleviate disease progression,and has a certain clinical value in the treatment of diabetic nephropathy.
