宫腔观察吸引手术系统在人工流产术中的应用

摘　　要：目的探讨宫腔观察吸引手术系统在人工流产手术中的应用效果及安全性。方法研究对象选择2019年1—6月银川市第一人民医院计划生育门诊行人工流产术的患者共425例,分为3个组,超导可视组(188例)采用超声可视人流仪,宫腔直视组(159例)采用宫腔观察吸引系统,普通人流组(78例)采用传统负压吸宫术。比较各组患者孕产次、高危因素、手术时间、术中出血量、并发症发生情况,随访患者术后阴道流血持续时间,月经恢复时间,经量减少情况。结果普通人流组患者产次高于宫腔直视组(P<0.05),宫腔直视组高危流产次数多于普通人流组和超导可视组(P均<0.01)。宫腔直视组和超导可视组手术时间均比普通人流组所用时间短(P均<0.01),宫腔直视组术中出血量较普通人流组和超导可视组少(P<0.05或<0.01)。宫腔直视组术中及术后并发症总发生率低于普通人流组及超导可视组(P均<0.01)。结论采用宫腔观察吸引系统行人工流产术安全,疗效好,值得临床推广使用。Objective To investigate the application effect and safety of uterine cavity observation and suction system in artificial abortion.Methods Subjects a total of 425 patients who underwent induced abortion in the family planning clinic of our hospital from January to June 2019 were selected and divided into 3 groups:superconducting visual group(188 cases):ultrasound visual abortion instrument was used.In the direct vision group(159 cases):the uterine cavity observation and attraction system was used.General group(78 cases):the traditional negative pressure suction uteri was used.The pregnancy times,high-risk factors,operation time,intraoperative blood loss and complications were compared among the three groups.The duration of postoperative vaginal bleeding,menstrual recovery time and menstrual volume reduction were followed up.Results The frequency of births in the direct vision group was lower than that in normal abortion group(P<0.05),but the frequency of high-risk abortion was higher than that in normal abortion group and superconductive visible group(P all<0.01).The operation time and intraoperative blood loss in the direct vision group and the superconducting visual group were lower than those in the ordinary abortion group(P<0.05 or<0.01).There was a large proportion of high-risk abortion in the uterine cavity direct vision group,but the incidence of complications was low(P all<0.01).Conclusion the use of uterine cavity observation and suction system for artificial abortion is safe and effective,which is worthy of clinical application.
