观察白芍总苷片联合糖皮质激素治疗系统性红斑狼疮肝损害的肝功能改变结果

摘　　要：目的:探究对系统性红斑狼疮肝损害应用白芍总苷片联合糖皮质激素治疗的临床疗效。方法:此次研究随机选择113例接受系统性红斑狼疮肝损害诊治的患者,依据患者应用治疗方式的差异将其分为常规治疗组(56例)和联合治疗组(57例),常规治疗组患者应用糖皮质激素类药物进行治疗,联合治疗组患者同时配合应用白芍总苷片治疗。对比两组患者的肝功能指标、中医证候积分、临床症状、免疫学指标检测阳性率以及不良反应发生率。结果:与治疗前相比,两组患者γ-谷氨酰转肽酶(γ-glutamyl Transpeptidase,γ-GT)、血清谷丙转氨酶(Alanine Aminotransferase,ALT)、肝功能分级评分法(Child-pugh)评分均较治疗前有显著改善,联合治疗组患者各项观察项目改善效果均明显优于常规治疗组(P<0.05);两组患者治疗前中医证候积分的差异无统计学意义(P>0.05),治疗后联合治疗组患者中医证候积分明显低于常规治疗组,且均好于治疗前(P<0.05);联合治疗组治疗总有效率显著高于常规治疗组(P<0.0068);联合治疗组各项免疫学指标检测的阳性率显著高于常规治疗组(P<0.05);联合治疗组不良反应总发生率明显低于常规治疗组(P=0.0049)。结论:对系统性红斑狼疮肝损害应用白芍总苷片联合糖皮质激素治疗可取得理想的治疗效果,不仅临床疗效有所提升,中医证候、免疫学指标以及肝功能指标均有所好转,且不良反应的发生率较低,安全性较高,值得推广应用。Objective:To explore the clinical efficacy of total glucosides of white peony tablets plus glucocorticoid on liver damage in systemic lupus erythematosus patients.Methods:113 patients were randomly selected,and were divided into the conventional group(56 cases)and the combined group(57 cases)according to the treatment methods.Patients in the conventional group were treated with glucocorticoid medicines,and patients in the combined group were treated with glucocorticoid plus total glucosides of white paeony tablets.Results:Theγ-GT level,ALT level,and Child-pugh score in the groups were significantly improved compared with before treatment,and the improvement of all observation items in the combined group was significantly better than the conventional group(P<0.05).After treatment,the TCM syndrome scores in the combined group after treatment were significantly lower than the conventional group,and in the groups after treatment were all better than before treatment(P<0.05).The total efficiency in the combined group was significantly higher than that in the conventional group(P<0.0068).The positive rate of various immunological indicators in the combined group was significantly higher than that in the conventional group(P<0.05).The total incidence of adverse reactions in the combined group was significantly lower than the conventional group(P=0.0049).Conclusion:Total glucosides of white peony tablets plus glucocorticoid on liver damage in systemic lupus erythematosus patients can gain an ideal therapeutic effect.Not only the clinical efficacy has been improved,but also the TCM syndromes,immunological indicators and liver function indicators have all improved.In addition,the incidence of adverse reactions is lower,the safety is higher,and it is worthy of popularization and application.
