自动弹力线套扎术联合套扎线远端高渗糖注射治疗混合痔的疗效分析

摘　　要：为探讨自动弹力线套扎术(RPH)联合套扎线远端高渗糖注射治疗混合痔的疗效,选择2019年3月至2020年3月我院收治的90例内痔为Ⅱ~Ⅲ期的混合痔患者为研究对象,患者均应用第4代自动弹力线痔疮套扎器行RPH,随机分为对照组和观察组,各45例,对照组术中套扎线远端不做特殊处理,观察组套扎线远端注射50%高渗糖注射液。比较2组手术总体疗效、术后肛门疼痛、水肿、出血、坠胀情况,以及手术时间、住院时间、术后恢复工作时间。结果显示,2组总有效率比较差异无统计学意义,P>0.05;观察组治愈率明显高于对照组,P<0.05。观察组术后24h、首次排便时及第7天肛门疼痛评分略低于对照组,但差异无统计学意义,P>0.05。2组术后肛门水肿发生率、手术时间、住院时间比较差异无统计学意义,P>0.05。观察组术后肛门坠胀、出血发生率明显低于对照组,且术后恢复工作时间短于对照组,P<0.05。结果表明,RPH术中套扎线远端注射高渗糖注射液可提高治愈率,降低术后出血、肛门坠胀风险,促进患者术后恢复。This study was to investigate the effect of RPH combined with hyperisotonic glucose injection at the distal end of ligation thread in the treatment of mixed hemorrhoids,enrolled 90 patients with mixed hemorrhoids ofⅡ-Ⅲstage inner-hemorrhoids into this research:patients all received RPH-4 treatment,and were randomly divided into control group(45 cases,non-management at the distal end)and observation group(45 cases,at the distal end injecting hyperisotonic glucose of 50%);then,compared both groups’total effective rate,patients’status on postoperative pain,edema,bleeding and anal tenesmus,as well as the time for operating,hospitalization,and for returning to work.As results,though the total effective rate of observation group was slightly higher than that of control group,but there was no significant difference(P>0.05);but in curative rate,in the incidence of anal tenesmus and bleeding,and in the time for returning to work observation group was significantly and respectively higher,lower,and shorter than control group(all,P<0.05);as for the score ratings on pain 24 hr.and 7 th day after surgery,as well as anal edema incidence,operating time,and hospitalization time there was no statistical difference between the two groups(all,P>0.05).Results show that RPH plus injecting hyperisotonic glucose at the distal end of ligation thread can lift curative rate,reduce risk of postoperative bleeding and anal tenesmus,promoting patients’postoperative recovered.
