摘 要:为探讨重组人干扰素α1b联合无乳糖饮食用于小儿轮状病毒(RV)肠炎治疗的临床疗效,以及对肠黏膜损伤、心肌酶谱的影响,将2017年10月至2019年10月我科收治的120例RV肠炎患儿随机分为观察组和对照组,各60例,2组均接受常规治疗,在此基础上,对照组肌肉注射重组人干扰素α1b,观察组在对照组基础上配合无乳糖饮食。比较2组临床疗效,以及治疗前后肠黏膜损伤指标、心肌酶谱指标的变化。结果显示,1)临床疗效:观察组总有效率(95.00%)明显高于对照组(83.33%),P<0.05。观察组患儿退热、呕吐停止及腹泻缓解时间均明显短于对照组,P<0.05。2)肠黏膜损伤指标:2组治疗后血清二胺氧化酶(DAO)、内毒素(EXT)和晚期糖基化终末产物(AGEs)水平较治疗前均明显降低,P<0.05;但与对照组相比,观察组治疗后DAO、EXT和AGEs水平更低,P<0.05。3)心肌酶谱指标:2组治疗后天门冬氨酸氨基转移酶(AST)、乳酸脱氢酶(LDH)、肌酸激酶(CK)及其同工酶CK-MB水平较治疗前均明显降低,P<0.05;但与对照组相比,观察组治疗后AST、LDH、CK及CK-MB水平更低,P<0.05。结果表明,重组人干扰素α1b联合无乳糖饮食用于小儿RV肠炎治疗不仅可提高临床疗效,促进疾病转归,而且可保护患儿心肌功能,减轻其肠黏膜损伤。In order to investigate the effect of recombinant human interferonα1 bcombined with lactosefree diet in the treatment of infantile rotavirus(RV)enteritis,and its influence on intestinal mucosal injury and myocardial enzymes,120 RV enteritis infants who were treated in author’s department from October 2017 to October 2019 were randomly divided into observation group and control group,60 cases in each group,and the 2 groups received routine treatment.On this basis,the control group was intramuscularly injected with recombinant human interferonα1b,the observation group on the basis of the control group with lactose-free diet,then the clinical efficacy,intestinal mucosal injury index and myocardial enzyme spectrum index before and after treatment were compared between the two groups.As results,1)Clinical efficacy:the total effective rate of the observation group was significantly higher than that of the control group(95.00%vs 83.33%,P<0.05);the fever,vomiting stoped and diarrhea relief time of the observation group were significantly shorter than those of the control group(P<0.05).2)The indexes of intestinal mucosa injury:the levels of diamine oxidase(DAO),endotoxin(EXT)and advanced glycosylation end product(AGEs)were significantly lower in the two groups than before treatment(P<0.05),but compared with the control group,the levels of DAO,EXT and AGEs in the observation group were lower than after treatment(P<0.05).3)Myocardial enzymes:after treatment,the levels of apartate aminotransferase(AST),lactate dehydrogenase(LDH),creatine kinase(CK)and CK-MB in both groups were significantly lower than those before treatment(P<0.05),but compared with the control group,the levels of AST,LDH,CK and CK-MB in the observation group were lower than after treatment(P<0.05).The results show that the recombinant human interferonα1 bcombined with lactose free diet in the treatment of children with RV enteritis can not only improve the clinical efficacy,promote the prognosis of the disease,but also protect the myocardial function a
