七叶皂苷钠联合骨肽注射液对老年跟骨骨折术后患者肢体肿胀及骨折愈合的影响

摘　　要：目的探讨七叶皂苷钠联合骨肽注射液对老年跟骨骨折术后患者肢体肿胀及骨折愈合的影响。方法选取湖南省职业病防治院2018年12月—2019年12月收治的老年跟骨骨折患者98例,依照摸球法分为对照组与研究组,各49例。所有患者均行切开复位内固定术治疗,对照组予以七叶皂苷钠片治疗,研究组在对照组基础上联合骨肽注射液治疗。2组均以20 d为1个疗程。比较2组临床疗效,治疗后骨痂生长情况评分,治疗前后胰岛素样生长因子-Ⅰ(IGF-Ⅰ)和血清骨碱性磷酸酶(BAP),并观察2组并发症发生情况。结果研究组治疗总有效率高于对照组(P<0.05)。治疗1个月、2个月及3个月后,研究组骨痂生长情况评分高于对照组(P<0.05)。治疗前2组血清IGF-Ⅰ、BAP水平比较,差异无统计学意义(P>0.05);治疗后研究组血清IGF-Ⅰ、BAP水平高于对照组(P<0.05)。研究组并发症发生率低于对照组(P<0.05)。结论老年跟骨骨折术后患者采用七叶皂苷钠联合骨肽注射液治疗,可有效改善患者肢体肿胀,促进骨折痊愈,减少皮肤并发症发生率。Objective To explore the impact on sodium aescinate combined with bone peptide injection on limb swelling and fracture healing of elderly patients with calcaneal fracture after operation.Methods A total of 98 cases of elderly patients with calcaneal fracture after operation were selected from December 2018 to December 2019 in Hunan Occupational Disease Prevention and Treatment Hospital,which were divided into control group and study group according to ball touching method,49 cases in each group.All patients were treated with open reduction and internal fixation,the control group was treated with sodium aescinate tablets,the study group was treated with bone peptide injection on the basis of the control group.Two groups were treated with 20 days as a course of treatment.Clinical efficacy,score of callus growth after treatment,insulin-like growth factor-I(IGF-Ⅰ)and serum bone alkaline phosphatase(BAP)before and after treatment were compared between the two groups,and the incidence of complications were observed.Results The total effective rate of the study group was higher than control group(P<0.05).After 1 month,2 months and 3 months of treatment,the score of callus growth in the study group was higher than that in the control group(P<0.05).Before treatment,there was no significant difference in serum IGF-Ⅰand BAP levels between the two groups(P>0.05);after treatment,the levels of serum IGF-Ⅰand BAP in study group were higher than those in control group(P<0.05).The incidence of complications in the study group was lower than that in the control group(P<0.05).Conclusion Elderly patients with calcaneal fractures were treated with sodium aescinate combined with bone peptide injection,it can effectively improve limb swelling,promote fracture healing and reduce the incidence of skin complications.
